How Long Should I Wait to Have Rhinoplasty After Having Fillers on my Nose?

I hv been using non-permanent fillers for my nose (tip & bridge) for abt 2 yrs now (4 mths in btwn) My last procedure for fillers was done on 2nd Sept. I want to explore Rhinoplasty instead as I am tired of getting d jabs n wld like a more permanent look. I m planning for d procedure in Mar 2012. Consultations scheduled end Nov. Is d time span good enuf for d effects of my fillers to wear off for d consult in Nov n procedure in Mar? If no, how long should I have to wait before I can proceed?

A: Timing of Rhinoplasty After Prior Nonsurgical Rhinoplasty

Hello Kauakea,

Thanks very much for the question, one I get very frequently from patients seeking a Nonsurgical Rhinoplasty. Temporary filler placed in the nose can improve the shape of the nose for 9 to 18 months, though the correction may be present even longer. Fillers in the nose seem to last longer than some other areas of the face (e.g. around the mouth), likely due to the fact that the nose doesn't move much with animation, talking, and chewing. There are also permanent fillers which can be used, though this must be performed very carefully and judiciously. 

Getting back to your question, it would be helpful to know which filler was used. If one of the Hyaluronic Acid (HA) fillers such as Restylane, Perlane, Juvederm Ultra, Juvederm Ultra Plus, or Prevelle were used, the filler can be dissolved at any time with a few injections of hyaluronidase. You could have this reversal performed and have your rhinoplasty consultation and computer imaging done the following day. If on the other hand, your physician injected a non-HA filler such as Radiesse, the results can last for over a year. In my experience, rhinoplasty can be more time-consuming after Radiesse injection as the tissue planes are a bit more "sticky." In any case, be sure to do your due diligence and consult with surgeons specializing in rhinoplasty.

If I Am Considering Rhinoplasty-- How/ What Would You Reccomend to Fix my Nose?

I know that I want the dorsal hump minimized and the overall size reduced, but would like to maintain the natural sharpness as to not look like a different person. How possible is this?

A: Natural looking results following rhinoplasty

Thanks for the question and for posting your photos. It is definitely possible to reduce the size of your nose and remove the hump while still maintaining a nose which is balanced with the rest of your features and natural in appearance. In your case, this could be achieved by maintaining some of the height of your bridge and avoiding overrotation (turning up) of the tip of your nose. The vast majority of my patients come in with very similar requests to yours: wanting to change the nose in specific ways while still maintaining the general appearance of your face. Well-performed rhinoplasty can absolutely accomplish this.

I would recommend seeking a consultation with a rhinoplasty surgeon who performs computer imaging, as this will help to ensure that you and your surgeon are on the same page in terms of your aesthetic goals. I spend quite a bit of time with my patients on this exercise during their consultations, to help them feel comfortable that I understand the look that they desire. It's also a great chance for me to let them know what would be realistic given their particular anatomy and skin type. These photos then are used as a guide or "template" during surgery.

More specific surgical considerations, such as which approach to use (open versus endonasal) and which rhinoplasty techniques are needed, would be dictated by the aesthetic changes desired, your unique nasal anatomy, and the surgeon's preference and training.

My Nose Looks Very Long 9 Days After the Nose Job.

I had an open rhinoplasty 9 days ago. I love how my nose looks from the front, but from the side it looks very long and pointy. The tip of my nose is very hard. The tip of my nose looks very long and pointy. Is it because the tip of swollen? Is the tip will ever come down and will make my nose look shorter. If not is it hard to fix it.

A: Changes in the nose following rhinoplasty

Hello and thanks for the question. At nine days following rhinoplasty, it is very early in the healing process. At this point the entire nose is swollen which causes the nose to appear larger than it will be in the upcoming days to weeks. Additionally, swelling in the columella (between the nostrils) and even the upper lip can cause the nose to appear more rotated (upturned) than it will as the swelling subsides. I advise patients to wait 10-12 days before returning to work or social activities if they want to "get away with" having undergone nasal surgery.

Depending on the techniques used during your surgery, I would expect the nose to drop to some extent and appear smaller all around. You should discuss your concerns with your surgeon and wait several months to a year to fully assess your outcome. There are certainly revision rhinoplasty maneuvers which your surgeon can utilize to address your concerns, if they persist at that time.
