A: Lifting a heavy breast

Thank you for the photos.  Based upon the degree of laxity in your right breast and the nipple position it seems more likely that you will need a lollipop or full lift.  The diameter of your implants is wide and much larger (heavier) implants will drop more over time.  Rather than going with a wider implant you may wish to consider a more projecting implant with the same diameter, essentially going from moderate to high profile. This will give a more prominant upper pole.

 In situations such as yours  it is mainly the natural breast tissue that is drooping beneath the implant.  I will often reduce the natural breast volume because it is the natural breast that responds more  to gravity.  In a perfect situation you would have identical implant volume and identical breast volume.  If you reduce some of the natural tissue you will have the opportunity to do this and have more similar implant volume.

Be sure your surgeon has a comfort level with this as the blood supply to the breast can be more tenuous in a lift/reduction patient with an implant already in place.

How Long to Wait Between Brazilian Butt Lift and Breast Lift?

I want to get a brazilian butt lift and a breast lift w/ augmentation. I want to perform both surgeries together but I know that will be painful. How long is the waiting period to perform both surgeries. Which surgery do I perform 1st? How long is the recovery period for each surgery. I have a flat booty and wish for a butt, not a nicki minaj butt but a nice butt for my body. No less than 800 cc,s due to the fact that I.m 5'9. My bra size is currently A bt want a C.

A: Brazilian Butt Lift with Breast Lift and Breast Implants.....?

Although the combination surgery that you are interested in can be performed at the same time, (and I have done this), it is a tough recovery and positioning issues may cause breast implant mal positioning or healing issues.  I recommend separating these procedures into two surgeries. Which you do first is up to you.  However, if you want them done close together, I would recommend doing your breast implants/lift first, followed by the BBL several weeks later.  After the first couple of weeks, implant position and wound healing of your breasts aren't likely to be affected by laying on your stomach.  I believe survival of the transplanted fat cells is a more tenuous issue.  If you are separating these surgeries by several months, it really shouldn't make any difference as to which you have done first.

As far as your request for 800cc..... There is absolutely no way for us to know if you have enough fat available to have that much injected or if you have enough room in your buttocks for that much fat.  You may.... or may not.  Be sure you consult a Plastic Surgeon with a lot of experience with this procedure as well as the combined breast implant/Lift procedure.

Should I Have my Implants Placed Under the Muscle and Get a Lift?

I'm 21 5'6" 115 pounds mother of 2. I had 450 cc high profile saline implants placed above the muscle. The size is exactly what I wanted. I opted not to get a lift & now feel my areola & nipple are far to low. The size of my areola does not bother me. I was told I could achieve lift & a natural appearance with overs & the size but I feel that they look VERY unnatural & my bra straps do not lay flat against my upper chest. Will they change any more? Should I have a lift & get them placed under?

A: This was a poor choice of procedures.

No Question, the correct thing to do is to move the implants to under the muscle as well as have a lift. I think this will give you by far the best shape, with the same size and a much longer lasting result. 
As far as the timing of the surgery..... I would remove your current implants as soon as possible before your skin stretches as the implants settle. Don't wait for things to worsen as they settle.    I would move the implants to under the muscle and perform the lift simultaneously. This can be done safely if it is done carefully and using a proper lift technique.
