During a Full Face and Neck Lift Which is More Effective: SMAS Plication or SMAS Trimming?

Direct sutures or suspension (NOT thread) sutures into the SMAS? What has been your experience long term?

A: SMAS - Let the surgeon choose the technique

You are not really asking the appropriate question.  There are many maneuvers that can be done on the SMAS layer, and the decision making process is much more complex than "which is more effective."  For instance, in someone who has a very thin face where I want to maintain as much facial volume as possible, I am more likely to perform imbrication / plication on the SMAS layer rather than any excisional technique (SMASectomy or advancement flaps).  For someone with a "full" face, a SMASectomy may be the ideal technique, as it allows for volume removal, and prevents widening of the face.

You should visit a surgeon who customizes their procedures to fit your anatomy, rather than a following a cook-book procedure.  Allow them the latitude to make appropriate intraoperative decisions.  If you generally like the results they display in their photo galleries, you should feel comfortable that they will make similar appropriate choices when it comes to your surgery.

Best regards,

Can Facelift Affect Skin Tone?

Will a deep tissue lift or SMAS resection plication affect the skin color or tone and is there any skin bunching around incision site?

A: Facelift and Skin Tone

A facelift will not generally affect skin tone. Most facelift approaches will not result in bunching around the incision but I would certainly bring this concern up with your physician.
