Dr. Matthew Richardson is a double board-certified plastic surgeon based in Frisco, Texas. He is one of only a handful of surgeons who are dual fellowship-trained in facial plastic surgery and hair restoration surgery. His philosophy is that patients should always look natural, and he strives to help them achieve the best version of themselves at his private practice, Texas Facial Aesthetics. Dr. Richardson received his medical degree from the University of Texas Medical School in Houston, then completed a residency in otolaryngology (head and neck surgery) at the University of Mississippi Medical Center in Jackson, Mississippi. After that, he was selected through the American Academy of Facial Plastic and Reconstructive Surgery to complete fellowships in facial plastic and reconstructive surgery and hair restoration surgery with Dr. Daniel Rousso in Birmingham, Alabama at the Rousso Facial Plastic Surgery Clinic. He is certified by both the American Board of Facial Plastic and Reconstructive Surgery and the American Board of Otolaryngology - Head and Neck Surgery. He is also a member of the American Academy of Facial Plastic and Reconstructive Surgery. Dr. Jordan Cain is a facial plastic surgeon practicing at Texas Facial Aesthetics in Frisco. My overall experience with Dr. Richardson was top-notch. I have had a robotic hair transplant in NY previously, and the WAW FUE system that Dr. Richardson uses is superior imo. Q: Facial massage/exercise after facelift. What is and isn't safe to do and when?

A: Restrictions after face and neck lift are most important in the first few days and weeks after surgery. During that time, tissues and incisions are still healing and are more delicate. Typically, I don't have patients start to massage anything until at least 4-6 weeks after surgery if any massage is indicated, and massage should always be gentle and performed in the appropriate way. Early and aggressive massage could conceivably cause an incision to open or a suture to break, and this may be what your surgeon was referring to regarding "massaging away your facelift." Increasing your heart rate or blood pressure significantly with exercise such as HIIT workouts in those early days can also increase swelling or even cause blood vessels in the area of surgery that are still healing to rupture and cause bleeding underneath the skin. I have patients wait for 2 weeks before restarting any exercise, and I tell them to slowly work back up to their previous activity levels over a period of 1-2 weeks. At 6 weeks after surgery, most tissues will have healed with enough strength that restrictions should be minimal. In the long term, a facelift that was done appropriately by repositioning the correct tissue layers with the right technique should not be affected by sleeping position, running, jumping, plyometrics, or any other form of exercise. Each patient heals at a different rate, and it is important to always follow up with your surgeon regularly in the postop period for advice specific to your case. Hope that helps!

Q: I have quite a long face and I was wondering if forehead reduction would be good, or if you’d recommend something else?

A: Forehead reduction surgery is a good option for reducing the forehead size in a patient with a longer face and a large forehead, as long as proper facial proportions are maintained. I would not recommend having any surgery performed in order to improve any personal or professional life situations. I encourage my patients to have surgery only if they desire to do the procedure for themselves (not for anyone else) and have the resources (such as adequate friend/family support, time off work, financial resources, etc) to have the procedure comfortably and without undue stress. Q: Can you reverse hairline lowering?

A: Reversing hairline lowering is very difficult, if not impossible. There will most likely be scars and other undesirable cosmetic changes. This is why I commonly tell patients that I simply will not take the hairline beyond a certain point, and frequently turn away patients that may have unrealistic expectations or desire unnatural results. Removing 2 inches of forehead is an extremely dramatic change, and reversing that amount would be extremely difficult. 2 centimeters is a much more common amount of hairline lowering and might be easier to reverse.

A: I tell every forehead reduction patient that the scalp will be numb in the hair-bearing portions along the front for several months postoperatively. While long-term or permanent numbness is a risk, I've not had any patients complain about this beyond about 6 months after surgery. Most patients will report some occasional itching, tingling, "electric" sensations, etc in the scalp while the nerves recover. That being said, most patients do not complain about the scalp numbness at all postop. All the best,

A: If the hairline is lowered with forehead reduction surgery, most patients will have several months of numbness, sometimes taking even 6-9 months to return. If the hairline is lowered with hair transplants, the numbness will generally recover faster, and within 2-3 months, most patients are feeling normal again, but this is only an average.
