The consistent problem with direct neck lifts where neck skin is removed in the central portion of the neck is visible irregularities that can be very difficult to correct.
Initially when the skin is tight, the incisions will look good.
However, with time, the sides of the neck may relax and as they move forward the scar creates irregularities which show through on both the side and anterior views.
The standard neck lift does not have these problems and while more invasive, is certainly a better operation.

What alternative conservative treatment options should I consider before going for a neck lift to remove ugly skin folds after significant weight loss through exercise and diet? For someone who is still young and has mild hypertension, what options could be recommended?
Although there are many claims to neck skin shrinkage with many Machines, it is only effective for minor excess and seems to work best if you can liposuction the neck as well.
The typical excess skin and usually relaxed platysma muscle after weight reduction is truely a surgical problem.
In addition, patients who have had bariatric surgery and lost great amounts of weight may need what we call the "double lift". There is so much loss of elasticity that the initial lift may need a secondary lift touch up at six months in order get a really good result.

I am 50 years old and have noticed fat under my chin, slight jowling and some slight puckering/wrinkles in my neck. I have seen two plastic surgeons. One says I am a good candidate for neck lipo with fat injections to the pre-jowl area and a small sub-mental skin excision. The other said my skin is borderline and may sag after lipo. Both are Board Certified Plastic Surgeons. I don't know which opinion to trust more. Any help would be greatly appreciated.
We have done patients just as you with a combination of liposuction and laser.
The new fractionated CO2 lasera in experienced hands can be used for skin tightening of the neck.
This allows us to use liposuction in the neck at the same time as the Mixto in the entire face and neck to get some very nice results. It is done under just oral sedation and some local anesthesia and is less expensive than a surgical neck lift.
However, it does not give as dramatic a result as an invasive neck lift with tightening of muscle and removal of skin. The other important factor is that it does not get in the way of any future surgery.
