Facial Scars from Laser Hair Removal and Fraxel

I had several LightSheer treatments in the fall of 2003 to 2004 spring. This treatment worked fine but left hyperpigmentation and even caused some burns but they healed without scarring. In the summer of 2007, I had 1 Fraxel Laser treatment. It did not go well. I had 1 Yag laser hair removal treatment about 4 months later that fall. I have brown skin (Indian) so I had low settings. The Yag laser hair removal was great, but I began to notice depressions in my skin. They were everywhere. I've noticed my left side cheek is riddled with small follicle-sized holes. I can tell that my skin is of uneven depth on this side.

The difference between my left and right cheek is astonishing. Yet, I had both laser treatments on both sides. The technician sprayed cool mist on my face between passes. Is this the cause, and is it irreversible? Can dermaroller or needling help?

A: Depressed scar options

Depressed scars in ethnic skin particularly resulting from prior laser surgery warrants a more conservative approach. I would recommend regular microdermabrasion and topical retinoid under supervision of a board-certified dermatologist.

Can Lightsheer Diode Laser Remove Very Fine Black Hairs?

I am thinking of having Lightsheer Diode laser. I am a bit confused because some people have told me that the Lightsheer Diode laser does not work on fine hair?

Is this true? I want my whole face lasered where I have fine black hairs and coarse hairs. Shall I treat only the dark thick hairs and not the fine black hairs?

A: Fine black hairs should be fine for hair laser removal

Hair laser removal using Lightsheer Diode Laser should do a good job for very fine black hairs. The key is the color of the hair shaft/hair follicle, more so than the caliber of the hair shaft as the energy of the hair laser gets preferentially taken by the pigmented portion of the hair follicle.

Is my Skin Good for Lightsheer Laser?

I had patch test with Lightsheer Diode Laser at 3 different laser clinics. Each clinic used the almost same settings. At the first two clinics, they shaved the area and applied gel before treatment. After treatment, my skin got red and some bumps appeared, but I didn't burn. But when I had patch test at the third clinic, electrologist didn't shave and apply any gel before treatment. When she treated the area, I burned and got blister on my skin.

When the similar setting was used with same laser, why did my skin react differently? At the first 2 clinics (which use Lighsheer frequently on clients), I was told that my skin is good for Lightsheer but at third clinic (which uses more GentleYAG on clients), I was told that Lightsheer is not good for my skin. Why is this?

A: Lightsheer Laser Hair Removal Therapy

You must be as well-shaven as possible prior to any laser hair treatment to achieve maximal clinical result as the hair shafts compete with energy of the laser (and we want most of the energy to be absorbed by hair follicles) and these residual hair shafts may be carbonized and can subsequently amplify the energy of the laser leading to blistering and scabbing. Protocol for Lightsheer needs to be more conservative for darker skin types, e.g. lower energy with greater pulse duration.
