Transcutaneous vs. Transconjunctival? (photo)

I am a 63 year old woman with fairly good skin tone but I am developing bags under my eyes. I went for two consults and got two opinions. The first dr suggested a lower lid transcutaneous blepharoplasty at a total cost of approx. $2600. The second recommended a lower lid transconjunctival blepharoplasty at a total of $3400. How do I choose and why the difference in costs?

A: Transcutaneous blepharoplasty would be a better option

Based on your photo, it appears you have significant orbiculars oculi laxity.  You would benefit from tightening of this muscle and suspension of it from your orbital rim (orbicularis suspension).  Your may also need lower lid tightening as well.  A formal consultation is required to see what would be best for you.  Warmest regards.

Dissapointed in Answers from Surgeon - How to Find the Best Plastic Surgeon for a Man?

My husband is almost 40 and has very deep set eyes & is experiencing sagging eye lids, deep and numerous lines in his brow and eye area. Had a "consult" with MD, was told that a brow lift was out of the question as it would make him look too effeminate. A very slight bleph was possible, but he had to talk him into that. He basically acted as if the only option for men was Botox. This is becoming costly. My husband is disappointed & feels hopeless. Is this true? This is my MD, so I am angry.

A: Find a surgeon you are comfortable with

Find a surgeon you  and your husband are comfortable with.  I always strongly recommend patients get second opinions if there are any concerns.  Not all surgeons are comfortable with cosmetic surgery on males.  Make sure you see before and after photos.  Warmest regards,

Can Eyelid Surgery Be Done Without Anesthesia?

Can bleph eyelid surgery be done without anesthesia?  If so, how does it work?

A: Eyelid surgery can be safely performed under local anesthesia.

Eyelid surgery, or blepharoplasty, can safely be performed under local anesthesia.  In my experience, upper eyelids can easily be performed under straight local (numbing injections) or can be performed with local with some sedation.  Lower eyelid surgery is often more complex than upper eyelid surgery and most patients prefer to have general anesthesia (go completely to sleep) for this procedure.  Having local anesthesia instead of general anesthesia can also offer a significant cost savings.  Warmest regards, Dr. Pippin.
