Why Would I Want Both a Dermatologist and a Plastic Surgeon to Have a Mole Removed?

If I'm just going to end up seeing a plastic surgeon to make the removal/scar area look right, then why see a dermatologist? Can the plastic surgeon send the tissue to a lab to check for cancer?

Shave the Mole or Remove It All by a Plastic Surgeon? (photo)

Hi, I have a mole that is located on my left cheek right above the mustache end. This mole is about to be removed by a plastic surgeon.I know I am going to have a scar and I have been researching the mole removal procedures and it seems this is the procedure that will leave the biggest scar out of all the other procedures. So now I am reluctant to remove the mole using this method and I am thinking about shaving the mole and have less scar? So what do you doctors think? shave it or remove it?

A: Best way to remove a mole Shave removal and excision with sutures are 2 ways to remove a mole. In either case the mole is almost always sent for pathological evaluation. Shave removal can have an excellent outcome if done properly as can excision with sutures. A conservative approach would be to shave the mole and evaluate the cosmetic outcome results. In a vast majority of cases this is can bring an excellent result. You can always later perform excision if the results are not excellent or the mole comes back.

Should a Mole That Has Mild to Moderate Cells Be Removed or Watched?

A: Moles with atypia

Moles with atypia fall along a spectrum of benign to melanoma. Mild atypia falls closer to benign while the moderate to severe  are closer to melanoma.  That being said, they are atypical and not normal and no one can tell you whether a mole with atypia will become a melanoma in the near or distant future. Conservative patients and their doctors will excise mild atypia. Some patients and their doctors will simply follow clinically the mild to moderate atypia, while almost all dermatologists will agree that severe atypia requires removal with clear margins. This decision comes down to the patient, the type of atypia and the ability to follow closely any changes in the mole. Your family history and prior history of any melanoma will also be critical in deciding how to proceed. Although this is a grey area for sure, your dermatologist will be the one who can best advise you how to proceed and any decisions should be made by you and your dermatologist.
