My nose has changed in the past 3 yrs due to untreated rosacea. It is now bulky, thick at the bottom. Do I have to break my nasal bridge in order to achieve the look of my original nose? Can I just reduce my dorsem, and nasal tip? I have included a before photo of my original nose and a picture of my current nose. thank you

Answer: Narrowing the tip Rosacea can be a tough and frustrating problem, leading to general swelling and inflammation. These changes are primarily in the skin, which does contribute the nasal appearance as it blankets the nose. When there is a specific cause for the observed changes (ie rosacea), it is always best to treat the root problem first. You appear to be fairly young, so it is important to consider that there are some changes that could have occurred with development. We typically see most changes during the teen years. If the bones themselves widened or the cartilages changed shape, that would be something that can be changed surgically. However, if it is just the skin changes, medical treatment would be the more ideal option. So the first step would be to consult with a well qualified dermatologist and treat the rosacea. Allow the treatment adequate time to work. Then, if there are still issues you would like to address, a consultation with a plastic or facial plastic surgeon that specializes in rhinoplasty. Hope this helps, good luck!

Answer: Widened Nose It appears that a lot has changed between the two photos. Your face appears fuller overall with a more pronounced nasal width, jaw width, and heavier brow, so I don't think I'd attribute the nasal changes to rosacea alone. Make sure that you've reached skeletal maturity before considering a rhinoplasty. A reputable surgeon wouldn't perform one before that point. To reduce your nose, it does appear that you'll benefit from osteotomies ("breaking the nose") in order to narrow your nasal profile. Tip modification will be helpful as well to achieve what you desire.
