Please advise on sub glandular or sub muscular implants. I am 5ft 4, just under 10 stone and want a natural look. There are so many pros and cons reading about whether to go under or over the muscle and I don’t know how to make the decision. My surgeon says I have enough tissue to go over the muscle. Please advise! Thank you xxxx

Answer: Breast augmentation Submuscular implants have more soft tissue coverage and a more natural look. You appear to be an excellent candidate for a form stable anatomic implant.

Answer: Breast augmentation, 295cc Hello and thank you for your question. There are several advantages and disadvantages to over vs under the muscle. Under the muscle has the advantage of lower capsular contracture rates, less risk of rippling, and better visualization with future mammograms. Most patients say that under the muscle also has a more natural look and feel. The size, shape, and profile of the implant is based on your desired breast size/shape, your chest wall measurements, and soft tissue quality. This decision should be based on a detailed discussion with equal input from both you and your surgeon. This entire surgery can be performed with a small incision technique. Make sure you specifically look at before and after pictures of real patients who have had this surgery performed by your surgeon and evaluate their results. The most important aspect is to find a surgeon you are comfortable with. I recommend that you seek consultation with a qualified board-certified plastic surgeon who can evaluate you in person. It is difficult to make a judgement on pictures alone but from what I can tell, there appears to be minimal breast tissue on the upper pole of your breast and I would thus suggest you would benefit from placing the implants under the muscle. I feel this will also aid in giving you a natural look. All the best!

Answer: Either under or over the muscle breast augmentation I prefer under the pectoralis muscle augmentation in the proper candidates. Muscle shape can influence the decision. If your pectoralis muscle is short and tight it is better to go above the muscle. Discuss this with your plastic surgeon. Good luck....
