How much de-rotation and length can be added to my nose in a revision?

Question

I want the tip of my nose come down and align with my nostrils. I also would want a straight profile instead of a slope, with more build at the bridge. Is what I want achievable and realistic?

We can't really say how much derogation is possible in your case based just on a photograph. A lot depends on how mobile your tip tissues are and whether you've had any prior treatment to the area to create scar tissue which would hinder tip movement.In general it is usually possible to counter rotate the tip and make the profile less concave like you're looking to do. I recommend getting a consultation to go over your concerns and your desired results. Revision rhinoplasty is a technically challenging procedure and I would suggest that you seek a surgeon that has a good track record for natural results. The bridge of the nose can be built up 3 to 4 mm which will also help de-rotate the a couple of millimeters. It's important to know how much cartilage is left on the inside of the nose for potential grafting purposes.

lengthening and de-rotating  shortened/upturned noses after overly aggressive dorsal resection is possible but requires experience, judgement and meticulous technique.... the only limiting factor is non-permissive scarred skin which is unusual and results from multiple surgeries/complicationsyou probably will require at least spreader/septal extension grafting and a fixed columellar strut graft.... dorsal augmentation is often best achieved with a wrapped diced cartilage graftgood luck
