Ultherapy can target three different depths: the Dermis (deeper skin), Fat + Connective tissue (Superficial Musculoaponeurotic System-SMAS) and Muscle. The Superficial Musculoaponeurotic System (SMAS) is located below the fat and is a strong tissue that envelops the face. It is made up of collagen, elastic fibers, muscle fibers and fat cells. The ultrasound energy produced during an Ulthera treatment, precisely and consistently heats the targeted tissue to 60-70 degrees centigrade, which is the optimal temperature for contraction and denaturation of collagen.The results from an Ultherapy treatment work in a two-step process. The first stage is the contraction and denaturing of collagen, which causes an immediate tissue tightening, and resultant lift. The new were protocol that we are using now and Ultherapy is quite effective, and in fact Ultherapy was once again noted that the best nonsurgical skin tightening treatment in 2016. it is important to have a realistic consultation to make sure that what you would expect are not surgical results because Ultherapy will never deliver surgical type result.
Ultherapy could potentially be used to reduce your jowls, but the result would be more subtle compared to the result that could be achieved with surgery.
I would recommend a combination of ultherapy for tightening and lifting the lower face and jowls in addition to a filler at the marionette lines and some along the jawline where the indentation is. My favorite filler for this area is Radiesse. The ultherapy should be done first then the filler. If filler is placed first, the ultherapy could break down the filler.

Sometimes, it needs to be combined with fillers also, as the face doesn't just sag or drop over time with gravity, but it also loses a lot of volume. It can take up to 6 months to see results from Ultherapy. Personally, I wouldn't repeat the treatment on any of my patients after only a month.
