Successful Revisions Done on Mohs Reconstruction Lip Area Following Mohs?

Eight weeks ago I had Moh's and reconstructive surgery between the upper lip and right nostril. I have railroad tracks and a "puffy pillow" at the bottom point of the Island Pinacle that have been present since the stitches were removed. Perhaps a combination of too tight and in too long on the stitches. They were taken out 6 days after surgery and that is probably appropriate for an area of tension as the mouth is. I understand that I should wait a year for revision. What can I do now?

A: Be patient with Mohs healing

It is far too soon to be concerned with the appearance of you Mohs repair at 8 weeks.  In my office I recommend gentle massaging and use of a bioCorneum scar gel- which also has an SPF 30.  If you still have concerns in 6 months, you may choose to undergo a series of skin resurfacing procedures with a laser- such as the Artisan -which has proven results for minimizing the appearance of scars.  As always, it is best to share your concerns with your surgeon.  Best of luck, Dr. Clevens

BCC on Side of Nose - Mohs or Laser Removal?

I have a BCC in the left side of the base of my nose.I m 33yo, my mother just removed the same BCC on the same place, with Mohs surgery. Results are great and healing process fast. After my research, I am facing a dilemma. Should I do Mohs or a laser removal (2mm around the BCC)? Is it possible with Mohs to transfer cancerous cells through the blood stream to other parts of the body or in the blood circulation?

A: MOHS Is the most effective option for treating skin cancer

With the Mohs Surgery, your surgeon provides the most precise method for removal of the cancerous tissue, while sparing the greatest amount of healthy tissue. For this reason, the Mohs surgery may result in a significantly smaller surgical defect and less noticeable scarring, as compared to other methods of skin cancer treatment. I recommend the Mohs Surgery for the removal of skin cancer in areas where maximum preservation of healthy tissue is desirable for cosmetic and functional purposes, such as the face.I may also recommend the Mohs Surgery for lesions that have recurred following prior treatment, or for lesions which have the greatest likelihood of recurrence.

Can Scar Be Fixed After Mohs?

Can it be fixed? Should it be fixed? The photo shows my left eye 10+ weeks after a skin graft following Mohs surgery to remove a basal cell lesion. Even with daily massage, the scar remains very hard and lumpy. The graft pulls down my inner eyelid to the point that I have to reach in with a q-tip just to clean my eye. In the photos, my nose appears crooked but it is not (or wasn't). The graft had all bunched up right after the exterior stitches were removed and I think that's why it's so lumpy.

A: Skin graft

~ 10 weeks out is still relatively early in the postop period for skin grafts.  It will improve over the next several weeks and months.  Revision is possible to help improve the appearance however.  If you are developing a medial ectropion then you may want to seek consultation with an oculoplastics surgeon.
