My new surgeon does tegaderm for 3 months then switches to silicone of my choice. I purchased embrace for my lollipop incision. This is a breast lift revision. My precious lift is atrocious and I was left with wide scars due to tension (I was told this by multiple consults I went on) Curious as to what other surgeons scar control protocol is, when do you have your patients start silicone sheets?

Answer: Scar Protocol after Breast Lift Thank you for your question regarding scar protocol. Every plastic surgeon has a different scar protocol, and every patient is slightly different in wound healing. However, once the incision is completely healed and there is no drainage, it is appropriate to start silicone treatment. My protocol usually involves silicone cream and gel 4-6 weeks after surgery. I typically try to continue this protocol for at least one year. If there are areas of hypertrophic scar, they may require microneedling, steroid injection of 5fu, Fraxel laser v-beam laser, and possible surgical excision. These are my silicone treatments, but every plastic surgeon has their variation on this theme. Silicone has been medically shown far superior to any other products like bio-oils or vitamin e. If you have any more questions regarding the breast lift or mastopexy, capsular contracture, or breast implants in general, please contact my office. Since your breast surgery is somewhat complex for an average plastic surgeon, please seek an experienced, board certified plastic surgeon with significant experience performing these types of breast lifts. Do your research. Make sure your chosen plastic surgeon has performed at least 100 breast lifts. Make sure they have the before and after pictures to prove it. Answer: Scar Management Thanks for your question. Working with majority patients of color, this is paramount in my practice. Scar management at our office starts in the OR. With our suture technique and type of suture. We place tape on our incisions which stays in place for 1 month. After 1 month we recommend firm scar massage with Biocorneum silicone scar cream 2x daily. Finally, we recommend Morpheus 8 -/+ lumecca 3 months post op to assist with scar appearance. Good luck!
