I am a 29 year old Hispanic female. I had grade 3 ptosis before my breast lift 8 months ago and I am concerned that my breasts have not been lifted enough. Should I receive another breast lift and would implants improve my shape? Overall I’m quite disappointed with my results and very insecure about my breasts. Thoughts?
Answer: Considering Breast Revision for Augmentation and Lift Outcomes Thank you for your question regarding the shape, position, and overall result of your breast augmentation and lift. From your description and photos, I believe a breast revision would be the most beneficial in your case. A likely solution could be the placement of larger implants and a thorough revision of your breast lift. It appears that your implants need to be repositioned higher and the scars thoroughly revised. It also seems like you may have developed a keloid reaction to the surgery or the scars, which should also be addressed during the revision. It's crucial to ensure you work with a surgeon with significant experience in breast surgery for such a comprehensive revision. Thank you for your question regarding your breast augmentation and lift results. I hope this provides some clarity about potential next steps. Sincerely, Dr. Katzen

Answer: Hypertrophic scarring It appears that you have hypertrophic scarring around your incisions. This may be one cause for why your nipples have been pulled down to the bottom of your breast again. It is tough to know why this has occurred but usually it occurs due to excessive tension on her breasts. I would start with steroid injections and allowing time for the scarring to settle before deciding on a revision, although I think that you may need want to place her nipples in appropriate position on your breast. I think that this would be a difficult revision to achieve the perfect result but I am sure improvements can be made.

Answer: Lift Your nipples can be raised higher to give a better lift. the inferior incision would not be needed this time, or not as much to achieve this. You also have some thickening of your scars which can be treated. Please discuss your concerns with your surgeon.

Answer: Do I need another breast lift? Hello @gables, thank you for your question and showing your pictures. In order to improve the overall shape of your breast, you may consider a breast reconstruction with an inverted J scar for minimal scarring to enhance volume in the first third of your breast and to reposition the height of the nipples. An accurate surgical plan requires a consultation. The best course of action is to consult one or several board certified plastic surgeon for more information and options tailored specifically to your needs. Best wishes! Alan Gonzalez, MD FACS.
