Hello, you probably don't need the fleur-de-lis, nor do I see so much skin in the vertical component, but what you do need is a circular tummy tuck, you have to remove all the excess skin

It appears you would do well with the traditional extended Abdominoplasty. It’s hard to tell with shorts on but the incision would need to go from hip to hip, possibly a little further past to get a nice seamless, taught look. Again, the shorts make it hard to tell, but the scar would be low along your underwear line anyway and it beats the FDL anyway you slice it. I do not recommend a fleur de lis. The fleur de lis is necessary for excess horizontal laxity above the umbilicus. I recommend an abdominoplasty with mesh reinforcement. The mesh creates a more flatten abdomen by compressing the abdominal wall resulting in early satiety and helps maintain a reduced weight. After 6 months, you can consider a circumferential lift, if needed.

Congratulations on your weight loss of over 100 pounds. Great job! Do not do the FDL. You do not need a vertical scar. If performed correctly, you only need an extended tummy tuck or a 360 circumferential body lift. If the scar is placed correctly, the skin can be tapered horizontally. So you do not need a vertical incision. If you want even more definition, you could go back to a second procedure and have VASER Hi-def liposuction to define your abdomen. Many experienced board-certified plastic surgeons specializing in plastic surgery after massive weight loss found that abdominal etching after an extended tummy tuck or 360 circumferential lower body lift in massive weight loss patients yields incredible results. Hello, as seen in the photos, a good result can be achieved with the extended tummy tuck operation. The addition of a vertical incision can create a much tighter skin, but if you do not want scars, an extended tummy tuck in this area also gives a good result.

The vertical may get you some tighter in the upper abdomen, but you will have a long noticeable scar. If you go with the extended, you will probably get good results and can hide that scar in a bathing suit on the beach. For men who want to look good on the beach, I recommend the extended. Minimally invasive radiofrequency like Renuvion can be done later to give more upper abdominal tightening if needed.
