Hey experts! I, 33 y/o male, lost over 100 pounds and I now want to remove my excess loose skin in the abdomen and maybe some in the back. I’ve had 2 consults with board certified plastic surgeons and 1 recommend an FDL as he stayed with the additional laxity in the horizontal plane, that I wouldn’t be satisfied with my results otherwise. The next surgeon recommended an extended. Obviously I don’t want a vertical scar, but I’m not really sure how to choose with differing opinions.

Answer: Incisions Congratulations on your impressive weight loss! The FDL incision pattern is designed to address lax skin in the upper/middle portion of the abdomen. The extended tummy tuck will move the abdominal excess skin lower but you may have some laxity on the upper abdomen that remains. Your photos don't really depict how much loose skin you have on the upper abdomen to advise what is recommended for you, but in most cases the extended tummy tuck is sufficient. I've included a video on plastic surgery after weight loss that I hope you find helpful.

Answer: FDL or Extended Tummy Tuck for Massive Weight Loss Thank you for your question regarding whether you need an FDL or an extended tummy tuck after massive weight loss. Thank you for your history and your pictures. Congratulations on your weight loss of over 100 pounds. Great job! Do not do the FDL. You do not need a vertical scar. If performed correctly, you only need an extended tummy tuck or a 360 circumferential body lift. If the scar is placed correctly, the skin can be tapered horizontally. So you do not need a vertical incision. If you want even more definition, you could go back to a second procedure and have VASER Hi-def liposuction to define your abdomen. Many experienced board-certified plastic surgeons specializing in plastic surgery after massive weight loss found that abdominal etching after an extended tummy tuck or 360 circumferential lower body lift in massive weight loss patients yields incredible results. Thank you for your question about needing an FDL or an extended tummy tuck. Sincerely, Dr. Katzen
