Typically the male eyebrow sits at or slightly above the bony rim of the eye socket. The eyes may appear hooded or heavy due to a combination of either the brow complex falling below the rim or an excess of upper eyelid skin. In person evaluation will help to determine the cause of heaviness in your case and the best treatment plan. If your main issue is excess upper eyelid skin rather than brow descent, a conservative upper belpharoplasty (removal of eyelid skin) typically will not over feminize the eye and may give you a good result.

Male brows tend to sit slightly lower than female brows. Your brow appears to be in a good position. Elevation of your brow to try to smooth out your upper eyelid skin would likely result in a slightly surprised look and maybe over feminized your appearance. I believe a well done upper blepharoplasty would take care of the redundant skin of your upper eyelid with a very well camouflage incision line, and it would not alter the appearance of your eyes in any way, but to the better. you can definitely consider a browlift, and yes a temporal lift will focus more on the lateral part of the brow (away from the nose, more toward the side).

Based on your photos, you could be a candidate just for upper lid surgery. The question to answer is do you feel that your brows are lower now than they were in the past. If not, just do the upper lid surgery because you don't want to change your natural appearance too much. If they have drooped, then doing a browlift at the same time would work well. Be sure to see a facial specialist for consultation.

I woukd recommend a temporal browlift combined with a small skin excision from the upper eyelid .It will not change your eye appearance if properly done. Rely g on a browlift alone will mean raisin g the brow to a level that may not look natural.

You might wish to check out the Ten Minute Eyelift, which is performed using only local anesthesia and utilizes low intensity electrocautery to shrink the excess skin leading to retraction of the upper lid excess skin and hence greater exposure of the upper lid. I have been performing this procedure for many years with gratifying results. The resulting scars are hidden under the upper lid skin. The procedure, as its name suggests, literally takes five minutes per side and healing occurs over a ten to fourteen day period with typically little to no significant downtime.

Your photographs are limited. You would benefit from a brow lift. It appears your eyebrows are at the level of the orbital rim or slightly below it and you may benefit from raising them slightly higher. This will decrease the vertical height of the forehead and lower the hairline. Most of your excess eyelid skin is lateral and you would benefit from an upper blepharoplasty.

Hoodedness of upper eyelids can be caused by 2 different factors:
-excess of skin at the upper eyelid, specially after the 40s and 50s
-at any age, when the frontal complex and the eyebrows carried by it become droopy
-and a combinatio of both

In many cases it is a mix of droppy forehead-eyebrows plus excessive skin at the upper lid; should that combo be shared roughly 50/50 then both techniques can be applied: upper blepharoplasty + forehead-eyebrow lift.

When one of both causes prevail, then the most cost/benefitial choice is that one's treatmente, like in your case which I tag as 20% excess of skin and 80% droopiness of forehead-eyebrows; so my advice is getting the forehead-eyebrows lift and only optionally you may consider addint the blepharoplasty too, however lift-only option is the most rewarding choice at the lesser economical cost.

Strong warning for the forehead-eyebrows lift: no threads, no hairline lift, no trychophitic lift, no coronal incision, no endoscopic lift, all the former are either atrocities, short-lived results, a scam or producing unacceptable scars; the choice is a supraperiosteal lifting of the upper two-thirds by means of short temple area incisions.

In my opinion, a well designed upper lid blepharoplasty is still your best option. A thoughtful blepharoplasty should not change your eyelid shape and the scar is hidden in your natural crease. Often a browlift can be combined with upper lid blepharoplasty, but often I find in young men that upper lid surgery is all they need. Neurotoxin could also be used instead of a browlift to help elevate the soft tissues around the eyes.
