How difficult is it to place a fascia graft on tip of nose? I have "paper thin skin"

I appreciate your question and would strongly recommend you select a plastic surgeon who has extensive experience in revision rhinoplasty to ensure the best possible result. While grafting of the fascia to provide subtle augmentation to the nasal tip is relatively common, the fact that you have thinner skin necessitates the expertise of a revision surgeon who has worked with similar situations in the past. An experienced surgeon will be able to produce very natural results. While the nasal tip may look round immediately after surgery due to swelling, it should become less bulbous as healing occurs. Thank you for the good question. It is a simple technique, and donor site issues in the scalp are rare. I would recommend it in revision rhinoplasties, and also in primary rhinoplasty where there is thin skin and cartilage grafts are required. Fascia graft revision for tip? Technically the procedure is not difficult in my 40 year experience. Fascia is taken from the scalp so you wlll have a small incision with no hair shaving. Make sure your surgeon is experienced and comfortable doing it. 

Fascia grafting to the tip is not difficult to do, but I would most certainly seek a revision rhinoplasty expert if you have have truly thin skin and need any grafting done.   You typically want to seek out a surgeon who does whatever you need commonly and is aware of the issues associated.  Rhinoplasty has a 5-10% revision rate quoted in papers written by the best surgeons it the country.  Real revision rates can be higher from surgeon to surgeon.  

 Technically, it's not that difficult to place fascia over the tip cartilages. A separate incision has to be performed in the scalp to obtain the fascia, and then this is draped over the nasal tip cartilages. Best to have this conversation with your operative surgeon to make sure that is really needed.

Adding fascia to the tip can help hide some irregularities. It’s not necessarily ‘complicated’ but it is kind of a revision as the nose needs to be opened. The fascia will swell and look round for a while. A consultation is important to see what you need and if this is a good choice for you. Best wishes
