I am 43, slender and in shape, but have the dreaded loose skin under my chin and on my neck. I tried thermage a couple years ago to help in tightening this area, with no real results. I want to do the least invasive procedure that will yield good results of making this part of me look better and younger . What procedure / surgery would you recommend? Thanks in advance for your input!
Answer: Submentoplasty Would Likely Work Thanks for asking this common question. Non-invasive procedures have limited utility for loose skin underneath the chin and neck. Submentoplasty is best for people who have significant amounts of fat or looseness of the underlying platysma, characterized by platysma bands. It also leaves a submental scar. In general, some version of a neck lift or lower facelift through an incision that is very well hidden around the ears can offer a sustained correction of 10 years or more, with a recovery of less than 2 weeks. The procedure can be offered under local anesthesia with sedation, and it can be done without an incision below the chin. That would be my tentative recommendation, although an in-person consultation would help. However, to get improvement in the skin laxity, which I see in the lower neck as well, a submentoplasty would be an inadequate solution. Good luck.

I do perform submentoplasty and neck lifting together for tha cases like you. It will help to lift your cheeks, the corners of your mouth, reduce jowls and marionette lines, as well as provide a significantly better neck contour. Answer: Facelift with necklift vs noninvasive options Thank you for your photos! Based on your photos the best option would be to have a facelift with a necklift. You have some obvious platysmal banding in your neck so I would recommend a platysmaplasty with your necklift to give you the smoothest neck appearance. In the end, your surgical options will give you the best results, but more importantly, it will make you the happiest. Also, your photos show that your lower face and cheeks have lost volume so any attempt at a surgical procedure without adding volume to your midface will result in recurrence of the marionette lines and nasolabial folds in your face in a year so adding fat or a synthetic volumizer at the same time would make your neck and face look much better. If you are only looking for a mild improvement, placing botox in the platysma muscle of your neck and using volumizers such as Radiesse and Juvederm Voluma in the face can give you an improvement without trying surgery though in the end putting that money towards surgery will be the most cost effective way of getting what you want long-term.
