I had thick skin and was very insecure about my nose. I honestly just want your opinion on how it looks because I get very insecure still. I wake up in the morning and it is so swollen. Will tht ever go away? Should I do revision based off what it looks like? Thank you.
Answer: Skin envelope is thick Prolonged fluctuating swelling can happen for years following reduction rhinoplasty in someone with thick skin. It is a difficult problem to treat with revision rhinoplasty. Sometimes the skin can be conservatively thinned (some fat removed) to allow for some better definition, but this can be risky if done too aggressively (lead to uneven skin appearance). Treatment with a course of low-dose accutane has been helpful for some patients by reducing the sebaceous component of the skin. A dermatologist could likely help you with that option.

This is a good example of a commonly seen post rhinoplasty concern in patients with thick skin. At the 3 year plus mark I don't believe this is a swelling issue. In some cases the skin can be thinned a bit, but with caution since this can result in diminishing the blood supply to the skin in that area and result in loss of tissue coverage, and, or visible contour irregularities. I suggest that you discuss this matter with several plastic surgeon experienced with revision work. Also obtain a full set of your images (PreOP, Post Op, and current) along with your operative dictation. Best wishes,Sergio

Answer: Revision rhinoplasty? Only you can decide if it is worth it to you. The tip can probably be made narrower--it is not swollen any more. See several very experienced revision rhinoplasty surgeons for an evaluation as well as your surgeon.

Have you ever tried steroid shots the tip of the nose for your thick skin? Blenderm tape is another option in addition to a low salt diet. It's best to go back to your original operative surgeon for follow-up. The follow-up appointments are just as important as the surgery itself to manage the soft-tissue envelope swelling over the cartilaginous framework of the nose.
