Hello,This is a complex issue that I see.  I treat such problems with either a onlay graft, or by adjusting the shape and, or, the position of the cartilage in that area.  I would refrain from filler as a "quick fix" secondary in that the skin envelope in this area is not as amenable to accept filler, and can compound the problem.  This is an  area ( the nasal tip) that is frought with complexity and unpredictability- particularly after a revision procedure.  I suggest that you address this with your primary surgeon, allow sufficient time for healing (typically 7 to 12 months), consider massage of the area if firm, and if an additional revision is required get several opinions from plastic surgeons that perform revision rhinoplasties.I hope this helps,Sergio Maggi, MDAustin Plastic Surgery Centerdrmaggi.com

Hi and thanks for your question and photos.  This looks like more of a surgical revision issue, but filler can be used to camouflage the problem area.  I would be happy to see you for this.

For minor depressions I have found fat grafting to be an effective and long term solution that only takes a few minutes.   About 50% of the fat by volume is retained, and survives a long time ( many years).  However, your unevenness looks pretty severe, and fat would soften it somewhat, but it would still be pretty visible. I think you will probably eventually need additional surgery. Fillers are also a possibility but are very temporary.

Cartilage grafting techniques are the mainstay for this type of issue. It's important to know how much cartilage is left on the inside of your nose for potential grafting purposes before undergoing a revision nasal surgery. We do not recommend injectable fillers in the tip of the nose

Try fat or Restylane injections. Thought the graft did not fill the enitre space, sometime scarring may be the culprit. Placing fat between the graft and the skin will fill the area and break any scarring that has formed. Of course, this may have to be repeated. You do not want the surgeon to overfill the area. It is easier to come back after 3-4 weeks if more is needed. It should last for at least one year or longer. Either way this is a simple fix and done in the office in a few minutes.
