Breast reduction (liposuction assisted or traditional) may be done in twilight and local anesthesia. Whether it is the way your surgeon/anesthesiologys would do it is an issue to be discussed with your surgon ahead of surgery. Good luck.

Liposuction of breast tissue to help reduce the volume of your breast is a known technique, although likely not done often because it may not improve on the aesthetics of the breast.
With liposuction, you may be able to remove some of the volume and weight, but liposuction will not help with the loose skin or sagginess of the breast. Liposuction will not reshape the breast to make them more round or perky. Nor will liposuction move the nipple areola complex in a more aesthetically pleasing position.
For functional and aesthetic improvement, you should consider a traditional breast reduction, where skin and breast tissue is excised, and your nipple areola complex is moved higher in position.
If the general anesthesia concerns you, then ask your plastic surgeon about his experience with his current anesthesia team and ask if you can speak directly to your anesthesiologist. They can provide you further information regarding general anesthesia and hopefully ease some of you concerns.

Hello, thank you for your question. I would recommend staying away from liposuction for a breast reduction. Yes, you can do liposuction to help contour the lateral sides but liposuction itself to remove fat is not a good idea. Part of the breast reduction is a lift of the breast, and you would not achieve this with liposuction. General anesthesia is a safe procedure when done with the right Dr. Make sure you are doing your part and finding a board-certified surgeon with a good team. Most breast reduction patient always state, "they wish they did this procedure sooner". Liposuction can be an effective method of reducing breasts and can be done with oral sedation and local tumescent anaesthesia. Also, given the fact you are perimenopausal, you can achieve significant success as the proportion of the tissue will be far as opposed to gland which cannot be aspirated with liposuction. Having said that, given the size of your breasts, a formal reduction will be more effective in rejuvenating the breast as the ptosis will be addressed as well as size reduction. I perform reduction by numbing the breast with a light propofol drip so no need for intubation or deep gas anesthesia minimising postoperative pain and nausea. All factors to be considered. The issue with liposuction of the breasts is that you are essentially deflating the breasts as you remove the fat. Because of this, liposuction of the breast is best used when the volume to be reduced is mild and you have good skin elasticity. Also, liposuction doe not reposition your nipple so if your breast as started to fall below the level of your inframammary fold, you will probably not love the look of your breasts after the procedure. Without knowing what your breasts look like it is hard to determine what the right treatment would be however breast reductions can be performed with twilight sedation without the need for general anesthesia.

Hello and thank you for your question! Yes, it is possible to have liposuction-only breast reduction! The reason this procedure is not more popular is for several reasons: 1) insurance usually does not cover this technique: 2) liposuction removes only fatty tissue and not skin; therefore depending on skin quality, there may be more/looser sagging skin after lipo-only breast reduction vs traditional breast reduction which removes skin, fatty tissue and glandular tissue; 3) liposuction is not good at removing dense glandular tissue, so for women with very dense breasts, liposuction may have more limited reduction power. The best advice is to seek in-patient consultation with a board certified plastic surgeon to see if this technique may be right for you.

Breast reduction can be performed with liposuction. Since the breasts have dense tissues, ultrasound energy should be used to eliminate fatty tissues from amongst breast tissues because of its ability to work around firmer soft tissues. This procedure can be performed by a provider who is experienced with providing local anesthesia and nitrous gas during your procedure thereby keeping you awake the entire time with little discomfort.
