I got a sliding genioplasty with a synthetic bone graft a year ago. The results are awful, I have a huge gap and step off in my labiomental fold. Along with the bad aesthetic results, I also have chin ptosis when I smile and have extreme tightness in my labiomental fold that’s unrelenting. I want a revision but I’m scared of lip ptosis due to my mentalis muscle being stretched out. Is this a real possibility? What are my risks?

Thank you for posting this. Your face is very well proportioned. There is a rule of lower facial thirds which says that in an attractive face, the distance between the nose and the mouth is half of that between the mouth and the chin. I would not recommend any surgery.

When not smiling, it appears that you have a short and slightly protrusive chin. There is a slightly deepened mentolabial fold. I would recommend a reduction genioplasty to reduce the projection and lengthen the chin in the vertical plane to diminish the depth of the mentolabial fold. This...
