When I had neck liposuction about 3 years ago my highly skilled and respected surgeon said it wouldn’t be possible to achieve an ideal aesthetic outcome because of my anatomy. Alas, my skin didn't shrink. I can't change my hyoid bone, but could I get a better result with submandibular gland resection? My last surgeon didn’t offer it as an alternative, but a recent consult with another very well regarded PS did. Do I look like a good candidate?

Answer: Face/neck lift with submandibular gland excision
Prominent or ptotic submandibular salivary glands are fairly common. They are not harmful and neither are they tumors. As long as they don't get infected or progressively enlarged, you can live with them.
I do not recommend using nitropaste prophylactically to try and avoid complications from smoking. It would be much better if you stopped smoking permanently. If this is not possible, I would recommend quitting at least two weeks prior to surgery and 2 weeks following surgery.
