I have been recommended 310cc teardrop implants unders to take me from a 34A to a 'natural' D. I was told that as I had very little breast tissue and because I was opting to look as natural as possible the surgeon would definitely recommend teardrop implants otherwise with rounds I would get a more fake look and it would look 'stuck on'. Worried about tear drop implants as I've heard there is a 2% risk of rotation and whilst that is low any added risk worries me! Can rounds give the same look?
Answer: Round Vs Tear Drop As mentioned, photos and breast measurements are needed for an accurate determination of what will best fit. Generally speaking, when someone is thin and has a small or A cup breast, the decision becomes how big versus how natural. There is only so much space in which a particular implant will be able to fit and also look natural. And yes, a round implant in the subpectoral position in a thin person with an A cup breast can and will look completely natural if a conservative appropriate size implant is used. I do this all the time. At times, the size may be smaller than desired, but it's all about what fits if you want a natural and aesthetic looking breast. In thin patients with A cup breasts, depending on their measurements, I often place a range of implants from 225cc to 275cc, and they look natural and wonderful. At times a bit larger will work. We work with alot of models who come in with A cup breasts requesting a natural looking cup size larger. Their implant size ranges from 200cc to 250cc, rarely a 275cc.I am not a fan of tear drop or textured implants. In reality, a round silicone implant, becomes teardrop shape if you hold it upright, as the silicone or saline displaces more to the bottom...producing a tear drop shape. Also, if a round implant turns one way or another, it is still round. A tear drop shaped implant, if one rotates, it will create asymmetry. The teardrop shaped implant is made textured to attempt to keep it from rotating. The textured surface adheres to the overlying tissue, a velcro like effect, making it less natural and more palpable in my opinion, especially in thin patients.Bottom line, a conservative size, smooth round implant in a thin patient with an A cup breast will create a completely natural looking breast.

It does appear from your photographs that you are very petite with very little breast tissue. Breast implants should do a great job in terms of increasing your size as well as giving a more shapely, fuller, and perkier appearance of your breasts. Which type of implant will ultimately be your decision, taking into account the look and feel of each. The bigger question, if you have decided to have a breast augmentation, will be the placement of your implants. With as little tissue that you have, I would fear that the implant may be visible with increased chance of rippling if placed in the subglandular position. You may consider a subpectoral placement of the implant along with consideration for the highly-cohesive gel "gummy bear" implant, which I believe may be a great option for you. Good communication between you and your surgeon of your expectations is warranted - choosing your surgeon wisely is the first step. Discussion of your wishes and having an honest and open dialog of your procedure is mandatory (e.g, implant type, incision, placement, need for lift, etc). I have found that photographs brought by the patient is helpful to get a visualization of the appearance you wish for in terms of size, shape, fullness, etc. In addition, your surgeon's pre and postoperative photographs should demonstrate a realistic goal for you. Once this has been accomplished, allow your surgeon to utilize his/her best medical judgment during the procedure to finesse the best possible result for you after preoperative biodimensional planning and fitting the right implant for your breast width.

In addition teardrop shaped implants are textured implants which have a thicker wall which can be more palpable and visible than are smooth-walled implant. Most importantly don't attempt to go to be. My fear is that attempting to achieve a D cup hole result in an extremely large implant that will look obvious when you're petite frame with little breast tissue. Please consult a board certified plastic surgeon who is very experienced in breast augmentation.
