Interesting question. A crescentic mastopexy could help in conjunction with additional volume either via lipofilling or implants to help reduce the sagginess - which is your goal. This requires a detailed consultation and assessment to really understand your goals and present options with their pros and cons. The amount of breast tissue changes proportional to weight, pregnancy, lactation, heredity, or hormonal changes. The technique I recommend is a lift using The Bellesoma Method. This will reshape your breast tissue creating upper pole fullness without implants, elevate them higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained. Later, fat transfers can be performed if additional volume is desired. Implants are not lifetime devices. 8% fail within the first year and 30% fail within six years. Fat transfers are permanent and incorporated into the body.

A board certified plastic surgeon will be able to guide you to the correct size implant measurements and desires measurements of the chest wall and breast imprint width as well as knowledge of all breast implant sizes and profiles should make this process easy.

my preferred lift is a lollipop lift when a periareolar won't do (ie greater than 2cm lift required). The lollipop technique was created by a Canadian plastic surgeon named Elizabeth Hall-Findley and treats the breast as a three dimensional structure instead of two dimensions (anchor - lift). Most importantly it does not require the horizontal component scars that the anchor-lift requires and breasts appear much more natural, lifted, and less boxy. Lastly, the lift lasts longer because the lollipop doesn't rely on the skin to hold the breast up, its actually breast tissue thats being brought together to support the breast and hence improves longevity. If you are considering a surgery, I would suggest you to consult a board-certified plastic surgeon. Only after a thorough examination you will get more information and recommendations.

Avoid the temptation for a crescent or donut lift. They won’t lift you enough.?you are a great candidate for the lollipop lift with implants.

I appreciate your concerns, however it does appear that you have options, but certain choices need to be made and ramifications understood.
It appears from your photos that you may able to achieve modest lift with just breast augmentation with higher projection implants utilizing so-called dual plan technique. This will minimize scarring but achieve only a modest lift. Fat grafting does not appear to be an option because you do not have much fat to transfer.
Crescent lift with implants is an option, but also provides only a modest lift. If volume is not an issue and only upper pole fullness and lift is your desire, then you may have enough breast tissue to achieve your goals with only a breast lift and no implants, however you must accept the scars. Finally breast lift with small implants would achieve maximal lift and fullness, but again scarring is a necessary evil. It is best to consult with a board-certified plastic surgeon in your area to explore the best options, including the pros and cons of each, to allow you to chose the best procedure for you. The answer to your question is a balance between what you would like to achieve eventually and the scarring and volume that you are willing to accept. During the consultation I spend a great deal of time working out just this.I think you have a variety of options:
1. an uplift on its own - would make the breast sit better but would need a vertical scar and you would be unlikely to keep upper fullness.
2. An uplift with implants - would give you an uplifted perkier shape with upper fullness but at the expense of a vertical scar and maybe bigger than you would like to be.
3. A breast reduction and implants - similar to above but this time removing a similar amount of your own breast tissue top the implant being used to give you upper volume and perkier breasts but a volume similar to what you currently have.
4. Fat transfer - would give you maybe 1-1.5 cupsize increase in breast volume and this can be used to shape the breasts but not to the extent it would fully correct the droopiness you have.
If I saw you for a consultation i would model these options for you using the Vectra 3d system so you can look at the difference in the final result and choose accordingly
