Estimating how much fat can be removed can be challenging. Based on your pictures, I don’t think you are at high risk for insufficient fat harvest in the hands of someone who has sufficient skill and experience. The numbers quoted don’t quite make sense. Typically flat harvested should be at least 2/3 fat and 1/3 watery solution. That means if 900 cc is harvested. You should end up with at least 600 cc of fat. 100 mL of fat is an extremely conservative amount to graft for body contouring. The smallest BBL I’ve ever done on body types like yours is 400 cc per side. When discussing Liposuction, you need to make sure you describe exactly what areas are going to be treated. This changes the outcome and potential fat harvest numbers. Personally, I usually treat the entire torso as a single anatomic Unit by doing that patients are left with an even thin, fat distribution with our transition zones between treated, and I’m treated areas. A full torso includes upper abdomen, lower, abdomen, love handles, waste, full back and under armor areas all the way to the side of the breast. My best guess is total liposuction aspirate would be about 1500 cc in your case which should give it at least 450 to 500 cc to graft on each side. On individuals who may not have enough subcutaneous fat for equality, fat transfer procedures additional areas can be added to increase fat harvest. Arms or inner thighs are usually the next area to be added in addition to the full torso. Delivering consistent quality, liposuction and fat transfer results is more difficult than most people realize. There are major differences in outcome based on what you choose as a provider. Liposuction results, and to some degree in fat transfer results should be considered permanent and irreversible. If done poorly patients can be left with undesirable outcomes, including being left disfigured. I cannot emphasize enough the importance of properly vetting plastic surgeons when choosing providers. In my opinion, the best way to do this is to have multiple in person consultations. I do not recommend patients have virtual consultations if at all possible. During each consultation ask each provider to open up their portfolio and show you their entire collection of before, and after pictures of previous patients who had similar body characteristics to around the period and experience provider should have no difficulty showing you a lots of pictures of people who look just like you. For reference, an experience plastic surgeon should have no difficulty showing you the before and after pictures of at least 50 previous patients. Being shown a handful of pre-selected images, representing the best results of a providers career is insufficient to get a clear understanding of what average results look like in the hands of each provider or how many of these procedures anyone surgeon has actually done. mastering liposuction with fat, transfer takes years of experience and doing thousands of cases. There’s more variation in outcome results based on provider selection for Liposuction than any other plastic surgery procedure. Being bored, certified in plastic surgery with years of experience, and overall good reputation that does not mean somebody has mastered either Liposuction or fat transfer. Defined the best provider I suggest patients have multiple in person consultations. There’s no correct number of consultations needed. The more consultations you have the more likely you are to find the best provider, get a good understanding of your candidacy for the procedure and realize what these procedures can, and cannot accomplish. Finding the right plastic surgeon is a significant commitment in regards to time and effort. You’ll be reminded of the outcome every day for the rest of your life. I suggest patients take the process of provider selection far more seriously than most people do. When, in doubt, slow down and schedule more consultations. Best, Mats Hagstrom, MD
