Had a breast augmentation in Dec 2021. Now implants seem to have fallen lower than expected. I’m wondering if they are bottomed out? The upper pole seems much less fuller than bottom. I had a dual plane with 525cc silicone placed. Is a revision needed?
Answer: Implants Yes, you have bottomed out from the weight of such large and heavy implants. It would be best to revise your lower pocket, perhaps use mesh for more support, and go smaller so this does not happen again. The 525 cc implant size is too large and heavy for your tissue. Otherwise, wear a good supporting bra in hopes it does not get worse.

Answer: Very Slight It does appear that your implants have migrated slightly lower, resulting in pseudoptosis and "bottoming out" to a slight degree. It does not look terribly unnatural as many women, especially those who have breastfed, will develop a natural pseudoptosis over time. Whether or not you get it fixed would depend on how much it bothers you cosmetically and whether or not it causes you discomfort.

I wrote the very first paper on the internal bra-capsulorrhaphy over 20 years ago. In your case, I would perform a capsulorrhaphy-internal bra and mastopexy. It doesn’t seem that they are bottoming out. I would recommend to consult with your original surgeon. If the implant are above the muscle, I would recommend to have a revision implants placed under the muscle
