Only if you were planning a very large implant could this be done without a lift, because a big implant fills the skin envelope on its own. In your case there is a capsular contracture, and removing that capsule will loosen your breast skin further; a contracted breast looks higher and tighter than it really is, so once the capsule is released the breast naturally drops and looks more saggy than it...
