In its most basic description, chin liposuction entails removing only the fat in the area directly under the chin. The overlying skin then adheres to the new contour as determined by the underlying muscle. Therefore, neither skin nor muscle tightening (platysmaplasty) is performed. In a neck lift, chin liposuction is usually performed, however, the underlying muscle is addressed as well via internal tightening sutures (stitches). If the muscle tightening is only performed under the chin, and no surgery is performed from the sides of the face (ie around and behind the ear), then this can be called a "submentoplasty" and/or "platysmaplasty". When the muscle and skin are tightened from behind the ears (with our without surgery under the chin), most surgeons would call this a "neck lift". Generally speaking, isolated chin liposuction is reserved for younger patients with tighter healthy skin. Once the skin and muscle start to lose their tone, then typically a more comprehensive neck lift is suggested. Depending on the skill and preference of your surgeon, both chin liposuction and a neck lift can be performed under local anesthesia in an office setting. Hope this helps clarify for you. Best, Doc L.
