Hello. I had a septorhinoplasty 6 months ago, but I notice a rather minor columella show which bothers me (my colummela was more visible before the nose job). I had a consultation with my surgeon and he told me that he could make my nose shorter during the revision rhino. Ive read here that hanging columella is fixed rather by trimming the cartilages and membrane, so my question is: will turning my tip up solve the problem? Is it possible to fix it with shortening of the nose only? Im confused.
Answer: Correcting a hanging columella. It is hard to give a meaningful answer without at least photos but best with a examination and prior op notes so what was done at the first operation can be understood. That said, a hanging columella typically is due to a elongated septum and/or excess soft tissue under the nose. Correcting these issue would likely have a better chance of retracting your columella than tip elevation of nasal shortening. Follow up with your plastic surgeon to express your concerns.

Answer: Hanging columella It is hard to comment specifically on your case without pictures. However, hanging columella is due to a rather long septum usually or poor columella support. Shortening the septum and rotating the tip does pull the columella up. You may also need columella graft to support it if it's week. Have to be careful if you already had septoplasty, to make sure you have enough support to your septum.

The upward rotation can fix a hanging columella to a mild degree and it is used primarily to lift a tip. If this was suggested to you perhaps that was the issue for you. Perhaps this is all you need, but it is very hard to say if this is the right procedure for you without firstly, seeing your photos in 6 views and also examining your nose in person. Typically the septum might need to be shortened along with the medial crura to address the issue. That would need to be determined on the lateral view and an examination in person.
