I scheduled my neck, face, eyelid and brow lift surgery with Dr Ridenour, without hesitation, after my son received a beautiful rhinoplasty from him back in 2015. You can expect tender loving care from the doctor and his nurses; beginning with your first consultation, and lasting throughout your complete healing process.This was MY experience:I didn’t feel completely comfortable being seen in public for the first month. After that though, I was “good to go”! Yes, there was facial swelling. Yes, there were many stitches. But, believe it or not, I experienced no pain from the surgery or the healing process! I felt “pressure” in my face due to swelling, and my scalp felt “tingly” (similar to what you feel when removing a tight ponytail). Both were “sensations” that I felt, but neither were “painful”.You will need a helper for the first week to assist with your after-care, and to drive you to frequent follow-up appointments with the nurses. My advice would be to take a 2 week “staycation”, to relax and tend to yourself. You can expect “complete recovery” to take 4-6 months, so plan your procedure strategically.Since my surgery (in 2019), I’m SO much happier with the contour of my neck, and I no longer have a need for under eye concealer make-up! Also, a week doesn’t go by without me receiving a compliment on the texture of my skin!My procedure was well worth the time and financial commitment. Don’t be afraid. Just do it!!Lastly, I also highly recommend the Alastin and Epionce skincare products available through Dr Ridenour’s office. They’re a game changer!I am so happy with my results! Many patients start out using a medical grade antiperspirant that contains an aluminum salt that works to block the sweat gland. However, these products are not always satisfactory to patients because of persistent sweating and local irritation from the product. Botox and Dysport are effective in the treatment of axillary hyperhidrosis, however, the cost of treatment can prove significant and regular maintenance is required in order to be effective. We prefer the Miradry procedure for our patients. Miradry is an energy based treatment that provides safe and effective elimination of sweat and odor in a majority of patients after a single treatment. A smaller percentage of patients will require 2 treatments for complete eradication of sweat and odor. Patient satisfaction is extremely high with a 90% "worth it" rating on this website. Neuromodulators such as Botox, Jeuveau, Dysport and Xeomin are effective in the treatment of axillary hyperhidrosis. However, the cost of treatment can be significant over time and frequent maintenance is required in order to be effective. We have chosen the Miradry procedure for our patients with excessive sweating. As mentioned by others, Miradry is safe and effective in eliminating sweat, odor and hair in most patients with just a single treatment. A much smaller percentage of patients will require 2 treatments for complete eradication of sweat and odor. Patient satisfaction is extremely high with a 90% "worth it" rating on this website. The downsides include minor issues such as temporary swelling and numbness. Infinity is a needle based radiofrequency device that lacks the time and temperature control needed to uniformly heat the sweat glands to the desired endpoint. It is difficult to say whether your concerns are the result of a misplaced or asymmetric fat graft or simply the result of normal age-related sagging in the lower face. A single two dimensional photograph is difficult to accurately assess. It is indeed possible to safely remove a small amount of fat by syringe liposuction using a very small micro-cannula, although minor irregularity is a potential complication. In some areas of the face it may be safe to use a fat dissolving agent such as Kybella, although this is an off-label usage and caution is advised. Finally, I would suggest that you step back from the situation and consider an overall re-assessment of your face. It may be that the treatment was simply inadequate to address your aging concerns. To rejuvenate the lower face and neck, an incision is required both in front of and behind the ear. The key to a good outcome includes preserving the position of the hairline, proper design of the incision in the natural creases around the ear, and closure of the incisions without tension or strain. Performing the operation at the proper depth of dissection also ensures a good blood supply. Hypertrophic scar should be rare following facelift surgery, even in patients that are of increased risk. Thickening of any part of the incision can be treated with serial injections of dilute Kenalog and 5-flourouracil with good success. My best, Dr. Ridenour
Thank you for your question and for providing your photographs. I think that your goals are well thought out and can be achieved with a secondary operation. I think that you will require a rib cartilage graft to provide sufficient strong cartilage for a predictable outcome. I would use a septal extension graft (SEG) of rib cartilage to lengthen and support the tip as the foundation for the operation. You will need additional cartilage grafts for a stronger nasal bridge, to support the middle third of the nose and to bolster the nasal sidewalls. My best wishes, Dr. RIdenour
