I went to Dr. Byrne in 2017 for a revision rhinoplasty. It was a complicated nose because I had a couple of surgeries before and the result was not optimal. In the first visit, he explained what needed to be done and was very clear about the surgery limitations and he said and I quote "I'm not going to fix it, I'll improve it". Well, I was skeptical at the beginning because of my not so good experiences with plastic surgeons and because several surgeons had advised against further surgery. I thought about it for a month and decided to give it one last shot. Here I am 2 years later and I'm so happy about my decision. He kept his side of the bargain and corrected 80% of my nose problems just he'd said in my first visit. I'm also very grateful for the fantastic staff, nurse, and admin in Green Spring Station. I highly recommend Dr.Byrne.
Getting a surgery is never easy. But from the very beginning I knew I was in good hands with Dr. Byrne and his staff. Everyone from his office, to his team of surgeons and nurses were extremely kind and professional. My nose turned out so amazing that my mother even cried after I got my bandage off. My breathing is better as well as my appearance. It’s like I have a new nose. And this is coming from a person who was born with a cleft lip. From the first meeting with Dr. Bryne, I knew he was the surgeon I wanted to perform my septoplasty and rhinoplasty. Dr. Bryne is kind, attentive and extremely talented. His professional staff made every effort to schedule my surgery at a convenient time, make payment issues as simple as possible and provide excellent care during my surgery and at follow up appointments. My overall experience was excellent. From the surgical coordinator, to the nurses, to Dr. Byrne himself to my wonderful results. Dr. Byrne and his office are extremely professional, listens to your concerns and provides the results you want. I went to Dr. Byrne for revision otoplasty. I received my first otoplasty from Johns Hopkins from the plastic surgery department and it was a horrible experience with a painful recovery and not satisfying results. After the original surgeon stated she would not operate again, I decided to get some other opinions. This led me to the 3rd opinion which was Dr. Byrne. I saw Dr. Byrne and went through the operating procedure a few days later. Dr. Byrne listened to my concerns, told me the realistic possibility of correcting the issue and ensured that his team was top of the line and I was in great hands. On the day of the procedure I met the entire team beforehand and they made me very comfortable. My recovery was ten times better this time and Dr. Byrne was able to answer any questions I had through the surgical coordinator almost immediately. I am now 4 months past my revision otoplasty and my results are great, no pain and the swelling has subsided greatly. I did not want to go through this procedure again but I am glad I worked up the courage to because Dr. Byrne changed the way I will feel every day for the rest of my life. I honestly can’t express how happy I am with my results. My nose is more beautiful than I could have imagined. It is completely natural looking. He put me at ease, and explained the process fully. I was always self-conscious about my nose growing up; I knew for a long time that I wanted to get a rhinoplasty. I had an initial nose surgery about three years ago with a different doctor and the results were a disaster (I guess that is what I get for not doing my homework). Dr. Byrne was recommended to me and I met with him a few times before deciding to go ahead with the surgery. I did it one year ago and honestly, he did an amazing job on my revision rhinoplasty. He listened to my specific requests and my complaints about the previous results; he then explained what could be achieved with surgery and the results. He took the time to even go a step further and explain why those results are the achievable ones for me (my septum, what the previous doctor did, my skin type, etc.). In addition, per a lot of research and reading, I quickly learned that a revision rhinoplasty is ten times harder than the first one especially since the doctor is working with someone else’s mistakes. Even with it being a more complicated surgery, the recovery was so much easier and smoother. Lastly, his office staff were super understanding, helpful, and friendly. I would definitely recommend him!
20 years ago I had trauma to the left side of my face which caused a permanent facial paralysis. After the incident, I saw many surgeons and had several reconstructive surgeries that did not work. I began to feel like I would never smile again. In August of 2018, I saw on the national news advances in surgical technology that made me decide to research surgeons and give it one more shot. After doing thorough research of surgeons throughout the country, Dr. Byrne's work and credentials stood out to me most. His before and after videos were evident of his talent and gave me the strength to setup an appointment. Three months later I flew out from California to Maryland for the T3 "Temporalis Tendon Transfer" surgery. The surgery went very smoothly and I was back in my hotel the same day. Looking in the mirror I could already see the improvement! It has now been six months and I feel like a new person. I only wish I could've done this sooner! I highly recommend Dr. Byrne! He has changed my life for the better.
Dr. Byrne and his entire staff were wonderful. I felt they understood and addressed all of my questions. I am a Surgical Assistant and work in the OR everyday, so I know how things should be handled. I was impressed and very comfortable at the Clarkview Surgery Center. My anesthesiologist, Dr. Isaacs was great, I had absolutely NO nausea after my procedure. My breathing improved immediately after surgery. My husband said I stopped snoring the first night despite all the swelling from the procedure. I have, and will, recommend Dr. Byrne to all of my family and friends.
I have just completed MOHS Reconstruction Surgery/ Forehead Flap for the tip of my nose. Thus far, the Team at Johns Hopkins have been exceptional. My private duty Nurse they recommended was also excellent. Patrick Byrne MD is very knowledgeable with an excellent bedside manner. Some surgeons routinely employ the use of packing, or plastic nasal stents, after surgery. Packing does prevent one from breathing, certainly. Some patients dont find this all that difficult, but others are quite uncomfortable. Personally, I dont routinely use packing nor splints, and find that the healing is just fine.
You are correct, that there are imperfections visible on your nose now. However, you look great! The reality is that there are always imperfections after rhinoplasty. There is no way to avoid them completely. Typically, they are visible when the nose is closely scrutinized. All human beings have facial imperfections. Other people generally dont pay attention to small details. Certainly, it is completely appropriate to discuss your concerns with your surgeon. Sometimes, it makes sense to try to revise an area to see if you can make it a bit better. I would just caution you however against pursuing perfection. It is a fruitless endeavor! Better to focus on what is good, and improved, and what you like. Good luck!
Skin grafts are occasionally a reasonable option to treat MOHS defects on the nose. When possible, we choose donor sites that match the skin characteristics of the nose optimally. No donor site is perfect, but we often use the forehead as a donor site. This location is well known to match quite well with our experience performing paramedic forehead flaps. It turns out that taking a graft high on the forehead, along the hairline (but not IN the hairline) can work really well. Perhaps the hard follicles in your skin graft can be removed by raising the skin graft as a small flap, and removing the follicles from the undersurface of the graft. Laser hair removal may work, if the hairs are dark colored. Electrolysis is an option as well. If all else fails, and if the hairs are really quite problematic, you could consider removing that graft and replacing it with an alternative. I hope this helps.
This is a common question, and I think there may be some confusing information online. The key difference is simply how one chooses to gain access to the structures of the nose. Does the surgeon make incisions only on the inside, and thus work through smaller openings? Or, does he or she extend those same incisions around the front of the columella (the little structure between the two nostrils) in order to get a wider field of access? The advantages of the latter are pretty obvious - greater visibility, probably greater control. I can tell you that after having personally performed over 2,000 such cases, there is no downside to the open approach. The incision heals incredibly well and so is a non issue. There may be a bit more swelling in the open approach in comparison to the closed, but this is of course temporary. Its important to note the each approach can achieve great outcomes (and, each approach can produce disappointing outcomes). More important is the question of what is done once you gain access to the structures of the nose. Here, there are also differences between surgeons as well.
There is always a certain amount of scarring that develops after rhinoplasty. The amount differs from patient to patient, and from case to case. After a year and a half, while there still may be some improvement over time, but it will not be much. Rather than focusing solely on the presence or absence of scar tissue, and the possible removal, I would instead encourage a discussion with your surgeons about options to achieve a result closer to your objective of a nice appearance to the tip. There are other maneuvers that in theory can be performed to achieve greater refinement. Whether or not additional surgery makes sense for you I cant say, but it may be more helpful to communicate what it is about your nasal appearance you dont like, and if there is anything that can be safely done to improve it.
