A closed rhinoplasty approach can address reduction of the bulbous tip. To reduce the bulbous tip requires suture techniques and a conservative cartilage removal applied to the lower lateral cartilages of the nasal tip. Defatting is not the answer to a bulbous tip. There is little fat there. Its just that the skin is thick and the cartilages are not visible as in a refined looking nose.Raising the upper bridge height, starting at the radix, will give the appearance of a better balanced nose, while in surgery, the alar cartilages, if thick and robust, may be narrowed to reduce the thickness of the tip.
