This 35 year old, fit professional came to our office to discuss the correction of a 4 years prior breast augmentation (with another surgeon). In the interim, she had lost 30 lbs over a 6 month period, which led to some unwanted changes in the breasts. The implants themselves, which were saline, had some visible rippling. She wanted Dr. Romanellis opinion on the best way to achieve the improvement she was seeking.  

Upon examination, Dr. Romanelli found her to be an excellent candidate for a remove and replace procedure, changing the implants in size (bigger) and to silicone gel.  After some discussion and trial sizing was done, they decided on smooth moderate high projection implants, 495 ccs. 

Surgery was less than one hour. Her smooth round saline implants, labeled 375 ccs (filled to 400 ccs) were removed and the new implants placed. The patients recovery was quite easy, as is the case with most remove and replace patients. This is due to the original stretch of the pectoralis muscle occurring mainly in the initial breast augmentation procedure.

Patient with prior saline augmentation. She had post-partum deflation with excess skin, lateralization of implants. She desired a tastefully augmented post-operative appearance. Her revision consisted of neo-subpectoral pocket (new implant pocket), lower pole wedge resection (of excess tissue) and new anatomic, form stable mentor memoryshape implants.

This patient had a breast augmentation from another surgeon and develop a severe deformity. I revised her augmentation by revising her breast augmentation. We removed her old implants and implant capsules. I developed a new pocket under the chest muscle as the old implants were placed above her muscle and had developed scar tissue which moved her right implant up and out.

18-24 year old woman treated with Breast Implant Revision

1 week post breast implant exchange left IMF repair. Patient went from 385 SRF to 695 SRF.

This patient had a deformity after an augmentation by another surgeon. Her muscle was divided too high resulting. in a visible deformity and asymmetry. I removed and replaced her implant. I repaired the chest muscle by sewing it back to her chest at its insertion points. Transitioned the implants to a subfascial plane which is over the muscle. I used an internal bra, Galaflex mesh, to hold the implants in perfect position. I gave her slightly more volume with her new implants.

Before and after removal and replacement of breast implants. This patient had aging subglandular implants that had become hard and deformed due to capsular contracture. I did a removal and replacement removing the old implants and capsules. I then placed the new silicone gel gummy bear SoftTouch implants in a new pocket under her chest muscle. I used a large size as she wanted more volume and shape.

45-54 year old woman treated with Breast Implants, Inspira Breast Implants, Natrelle Breast Implants, Breast Implant Revision

Before and after removal and replacement of breast implants. This patient had aging textured implants placed in the subglandular plane and had developed a capsular contracture. Her breast had sagged over the implant leading to a mismatch between the breast and implant. I did a full or complete capsulectomy removing her old textured implants and capsules. I made a separate pocket under her chest muscle, submuscular pocket, to place her new smooth round implants. She wanted a slightly larger size so we increased her volume by 80cc's. Her new implants should resist capsular contracture as submuscular implant have a lower capsular contracture rate. Her breasts and implants are perfectly aligned, soft, and non painful.

Woman treated with Breast Implant Revision. Patient had left implant deflation. We removed Allergan 550cc moderate style saline implants that were over filled to 600cc. They were replaced with Allergan Natrelle Inspira silicone high profile 650cc implants.

Breast augmentation REVISION with internal bra. This patient had older implants that began to sit lower over time. Additionally she didn't like the fact that they moved sideways under her arms when she laid down. I did a revision replacing her old implants with the newest generation silicone gel gummy bear implants. I also revised her implant pockets placing sutures to close off space laterally and to raise her breast fold about 2 cm. I then reinforced the new pocket with a Galaflex internal bra to prevent the implants from going back down or bottoming out again. Her new implant sit much higher and don't move to the side when she lying down.
