I hope this answer finds you well.First of my answer will be regarding your realistic expactation and from my point of view as a hair transplant surgeon.There can be some revision options but I will tell about 'my own best' treatment plan.1.Natural and good looking hairs is not possible without a suitable hairline design.Your hairline is more afro type of hairline(flat) and lower then it should be.(Da Vinci 1/3 rule).Despite the fact that SMP Works well in selected cases,for me it is not a good idea to do that in frontal hairline zone without any hairs.So it would be great if SMP in frontal hairline zone (1-2 cm upwards) and in temporal regions will be removed first to be able to re-create a natural (considering age,race,gender etc) one.2.Second step:HT. Your donor area is not really perfect but considering your expactation is realistic and limited with frontal zone,some natural result can be achieved.Frontal zone(not only hairline but all frontal zone) is approximately 70 cm2,and a good density is around 30 FU per cm2.so you will need 2100 FU grafts for frontal zone and a couple of hundreds grafts to restore temporal peaks.Your donor area can afford that.SMP with other areas (midscalp and crown) still is usefull espacially you have hair in frontal zone which frame your face in a very natural way.I hope it helps.Best Regards..
First of all everyone who thinks about HT must know about the main limitation in hair transplatation.Lifetime donor capasity is 'generally' between 6-8 thousand scalp FU grafts (LIFETIME!).Hair loss is progressive and you have a early onset of baldness,means it would not be a surprising if your baldness will get worse in future.It is very necessary to to have a long term plan for you too while trying to cover your current bald areas ..you must have donor reserve for rainy days in future.With your case (as far as I can see in the photos) 3500-3750 FU grefts would make a big improvement at the top if the implantations are done carefully with good delivery (in between existing hairs by using microscobe).I hope it helps..
Hair transplantation does not change the direction and the angles of your existing hairs.But while transplanting ,hair grafts can be implanted in any direction or angle(unfortunately sometimes in a wrong way artistically and scienticifally because of lack of exprience or concentration on the surgery)..despite the fact that there is general pattern in frontal zone about direction ,everybody is not the same.I personally follow the direction pattern of existing hairs (they are quiding me) so that the results are closer to natural..If the patients do not want the opposite of course).I hope it helps..
Despite the fact that any surgery can have some risks,(allergyc reactions etc.)Other than that there is only one life-threatening risk is that misusing the medications (overdose of local anesthesics or misuse of epynephrine) especially with the ones who has serious disease (hearth condition etc.).Hair transplant procesudures are one of the safest surgeries if it is performed by exprienced surgeon..Result are permanent if safe donor area is chosen for extracting the follicles.And of course all team must be very exprienced and skilled not to harm grefts in any stage of procedure..briefly safe and 'permanent' if surgeon and team obeys the medical rules..
For sure it can be fixed.Just my personal view as a hair transplant surgeon,unnnatural look is NOT only because of multiple hairs in frontal hairline zone but also not an artistic shape of hairline design applied such as man hairline should have had ''inverted V'' pattern at frontotemporal corners,midfrontal point should be lower than fronto-temporol point in vertical dimension,and there is no accordance with hairline and temporal triangle(peak).and your hariline seems flat (should have been a bit trianguler shape)...SMP does not seem to be capable to fix all these ,revision hair transplant would work very well ..I hope it helps.
