Placing filler in the tear trough would not be the best option for your case. Anatomically as we age our ligaments loose, this might cause filler migration and unnatural results. If, however you would like to try hyaluronic acid fillers, there is an option for placing the filler in the deep medical cheek pad, it would give support to the orbit and make the under-eyes appear less prominent.Second option would a treatment with polynucleotides, like Nucleofill, however you should keep on mind, that although we have obtained good results with polynucleotides, they are NOT FDA approved, and might not me available in every medical aesthetic providers office. Another option would be to stimulate the under eye area with PDLA, which helps restore the youthful appearance of the delicate under-eye area. If you are willing to tighten the skin of the upper lip, i would highly suggest trying Clearlift , non-ablative lip laser treatment to stimulate collagen production. It could potentially help reduce the appearance of the loose skin, also some patients notice improvement in the lip volume as well. The best way to approach the early signs of aging and loss of volume is to try a little bit of improvement in the lateral check area with hyaluronic acid fillers. This would give support to the ligaments and lift the face. I would restrain from placing filler in the piriform fossa, because in the longterm it might not look as natural. In your case, a little bit of support in the marionette area would also be beneficial. In addition to that, I will definitely recommend you to also try rejuvenating device procedures, such as laser resurfacing, or micro-needling, combined with RF to improve the texture of the skin and stimulate collagen.
