I got silicone implants in 2012 along with a lift. I was 23 at the time and I did the procedure due to having breast asymmetry and tuberous breasts. My implants came out much larger than I expected and stretched my areolas . I don't know what to do now in terms of what procedure I need and what type of results I can hope for. I want much smaller, perky implants and an areola reduction. Is there any hope based on what my breasts look like now? They are drooping with my nipples pointing down.

Typically, depending on how much smaller your desired implants/cup size are, the surgeon may have to do extra work on the capsule layer (implant pocket) to tighten it appropriately to better fit the new smaller implants. The areola can be reduced and moved up, along with having the remaining breast skin and tissues tightened through a Wise pattern (anchor incision) on each breast. Your best option is to have an in-person consultation with a board certified plastic surgeon who can do a physical exam, point out where the incisions and answer your specific questions, goals and concerns. The implants are removed, your breast tissue is reshaped creating upper pole fullness, elevated higher on the chest wall and more medial to increase your cleavage. Vertical scars are avoided, nipple sensation (in 95%) and the ability to breast feed are maintained if not injured during your previous surgery. The areolas can be reduced to the size you desire.
