I had a breast lift in 12/2019. Covid weight gain and loss, I have some sagging, but I am okay with that. I want to go from 32DD to a 32G. It sounds dramatic, but I purchased a bra and gel fillers and that’s what I want. I really miss how large my breasts were when I breastfed or was heavier. I had a consult with my previous surgeon and he said bigger implants would look bad given the vertical position of my breast. Can i do a teardrop shape instead of round to compensate for the position?
Answer: Implants No, going that larger will just give you more sagging. With such large and heavy implants, lifts will not hold. Please do not go larger. You will thin and stretch your own tissue and will just sag more.

Answer: Not larger implants Tear drop shaped implants are textured. Textured implants have been associated with BIA-ALCL, which is a form of cancer. Looking at your photographs, your breasts are almost touching your midline and your breasts are wider than your chest wall. Your breasts are mechanically unstable and adding more weight will make the situation even worse. I agree with your surgeon. Best Wishes, Gary Horndeski, M.D.

Answer: Implant type Dear andriadb, breast implant type and sizing selection is a complicated process and requires a detailed consultation that includes a discussion of your goals, a detailed examination, and a lengthy discussion. But the decision will always start and end with the patient. When it comes to sizing, we have all of our patients find 3 photos of their desired breast size. We ask for naked breast photos because clothing can obscure the true size (push up bras). Next, we consult with patients and go over the photos and take detailed breast measurements during a physical exam. Next, we using a sizing algorithm to determine a range of implants that will work based on the exam and the measurements.Lastly, during the operation, we use silicone sizers in the operating room to see how the size actually looks in the breast. We sit the patient up in the operating room, while asleep of course, and compare the sizer result with their desired look photos. If it is too small, we start the process over with a slightly larger implant. We don't stop until we get the perfect match.Of all of my breast augmentation patients, every one of them has been happy with their size due to this detailed and meticulous process. You don't want to invest time and money into a result that is either too big or too small. You should discuss your concerns with a board-certified plastic surgeon and make sure that your surgeon understands your goals. Answer: Large breast implants Dear andriadb, I am presuming that by "vertical position" of your breasts your surgeon is referring to the sagging or ptosis. Indeed, with larger implants and no breast lift, that could create more sagging, or a waterfall effect. On the other hand, it might give a little lift to your nipples if positioned well. Keep in mind that larger and therefore heavier breast implants tend to age more quickly over time (gravity). I am not sure that a very large tear drop shape implant would provide much benefit. Good luck!
