Question

32-year-old female, 5’6, 135 pounds and breast-fed three children. Work as a nurse and work out, active lifestyle. Initial implants with donut lift in 2019 with left side bottom out. Internal bar procedure on left and right implant replaced in 2020. Want to have left a areol more midline and think right breast is bottom out and flat? Breast issue hanging off of implant and flattened. Would you have fixed? Should it be fixed? Or just leave it? They look good in clothes, starting to Notice braless

Dear miamibabii305,
bottoming out occurs when the implant falls below the current breast crease or inframammary line. It's more likely to happen with inframammary incisions, which is why I try not to use the inframammary approach to any breast surgery.
Bottoming out can also occur if an implant is too large for the breast pocket. That is why it is important to choose an implant size that's correct for your body. I perform numerous measurements in the office to determine optimal implant size for my patient's bodies.
If you are concerned, please check it with your plastic surgeon for reassurance and to further discuss your options and possibility of revision.

If this bothers you, then have it fixed. You do need to wear a good supporting bra at all times while you are out of bed.

I'm sure your breasts look food in clothing, but I also see where you would like improvements.  
It does appear that you have some excess skin hanging below the implant on the left side and the issues with the nipple/areola placements.  It they really bother you, fix them.  If not, no worries.
A revision mastopexy (I might consider a lollipop) would move the areolas more midline and round out the breasts.
