Since you are wearing a night guard and still having symptoms, as well as noticing changes in your facial shape, it sounds like you would be a great candidate for neuromodulator injection, such as Botox or Dysport, into your masseter muscles. The neuromodulator will work to relax and eventually atrophy the muscle to alleviate your grinding, and with time, improve the bulkiness of the muscle along the angles of your jaw.
