Are my nipples too high after a revision? (Photos)

Question

I had over the muscle breast implants (260cc medium profile) in 2007. I had them redone in may 2025 and was told i also neeed an uplift. He went under the muscle and did 400cc high profile. When they settled, there seemed to be alot of issue excess tissue underneith and nipples seemed to be low and point outward. I went back for revision in feb just for the lift as the implants was in ok position. But now i feel the nipples are to high and come out of tops. Id say about and inch to high. Also the boobs dont move at all they seem very fixed and dont push together. Spoke to my surgeon he said he is happy with the results and purposley put the nipple in that position. Am i being unreasonable or can these be lowered? Is the risk gping back under the knife worth it? I also cant make a cleavage they dont bounce or move or anhthing but not sure if i can have overs as my last ones rippled. Thanks

From the photos you provided, I would do the following:
-Base Wedge Excision:  
  -The distance from nipple to base seems a little long (this would reduce it and also give you a little more upper pole projection)
  -This can also potentially pull the nipples down slightly so that they don't pop out of your swimsuits/dresses

Otherwise, your result actually looks nice.

Based on your description, it’s understandable why you’re concerned. After a breast lift, the nipples should generally be centered on the breast mound. If they remain noticeably high after healing, they may be positioned higher than ideal.

Breasts that feel firm and have limited movement can be related to implant placement, scar tissue, or your natural anatomy. Cleavage also depends largely on your chest width and breast anatomy, not only on the implants.

Your nipples are slightly too high and too lateral.  You may benefit from a donut mastopexy to reposition them more centrally and lower.

Certainly, your concerns are justified, and your doubts are understandable. The current situation looks clear from the photos and description.
Current Status
• The nipples (areola) are in a really high position and the upper pole is quite full.
• Breasts are very stable and mobility is limited (“tight” appearance, which is common in the early period in submuscular implants).
• Slight rippling marks are visible.
• Scars around the areola (periareolar technique).
My suggestions:
• Use Contractubex or similar silicone-based scar cream regularly (2-3 times a day, with a gentle massage).
• If you smoke, quit immediately &mdash; slows down healing and worsens scar quality.
• Drink plenty of water, eat healthy and do not disrupt the massage program given by the surgeon.
Are these normal?
Yes, largely early findings.
The "Drop & Fluff" process in submuscular implants lasts 3-6 months (sometimes up to 9-12 months). During this time, the implants sit down, the lower part fills, the upper pole softens and the breasts gain more natural movement.
Nipple height may be your surgeon's conscious choice. Your discomfort is completely understandable.
What Can Be Done?
1. Give time &mdash; Expect at least 6 months, ideally 9-12 months. Most patients get great improvement.
2. Massage + bandage &mdash; Apply regularly as prescribed by your doctor.
3. Revision &mdash; If your complaints still persist after 6-9 months:
• Lower pole expansion / capsule release
• Implant replacement (profile or size adjustment)
• Rarely nipple revision
Don't think about turning under the knife right now. Early revision can be risky. Being patient is the right approach.
Time is also the best medicine for rippling. It is expected to decrease as the tissue envelops the implant. If necessary, a fat graft can be considered in the future.
Last Word
Share these photos and feelings with your surgeon openly. You can also ask for a second opinion, it doesn't matter.
Don't worry, everything will be much better over time. Be patient, if you feel any changes in the process, write immediately.

Your nipples are placed too high and lowering them isn’t really an option.
The implant choice was too big in my opinion.
The implantdiameter is much larger than your natural breast diameter.
You also have divergent breast at point out to the sides, and this is made more with augmentation.
 The more projection the implant gives the more the divergence is put on display.
At this point, it’s going to be very difficult to your outcome.
I would be reluctant to have more surgery unless you can find a very talented breast surgeon.
It probably shouldn’t be thesurgeon who did your last procedure.
