Implant Exchange and Possible Lift - Is It Possible to Achieve Closer Cleavage? (Photos)

My primary goal is to reduce the gap between my breasts and achieve closer cleavage. My second priority is improve the position of my nipples by elevating and centering them. I would prefer to downsize my implants as my current 405 moderate profile feel too large in clothing and make me feel much bigger than I am. Based on my anatomy, is it possible to achieve closer cleavage with smaller implants? If a breast lift is recommended, what type is appropriate? The first photo was taken before my augmentation in 2020 and the second is current. My base width is 12.5
