Dr. Frojo is an exceptional surgeon. I had pondered getting a rhinoplasty for a few years and wanted to stay close to family if I decided to go through with it. I had a consultation with another surgeon prior to Dr. Frojo, but as soon as I consulted with him I knew right away he was going to be my surgeon. He listened to what I wanted and really honed down and understood what my ideal end results were. I wanted a more natural outcome which I feel he does amazingly when it comes to rhinoplasty. I am only 3 months post op so I still have a lot of healing, but am very happy with my results thus far. His bedside manner is phenomenal. I have seen him about 8 times in the last 3 months and he is always there when I have a question or want to see him. He also was kind enough to add on a procedure for me for free, he is a very fair doctor!
I had a gastric bypass back in February of 2022 and lost 125 lbs leaving a substantial amount of loose skin. I had never had plastic surgery before but was looking at major reconstructive work. Dr. Frojo performed a body lift and breast lift in May of 2023 and then did my thighs, an abdominal revision, breast augmentation and liposuction in July of 2024. I have never felt better and yes, I have shown my results to everyone and have sent new clients his way. Top notch work and I highly recommend him and his team. I will never go anywhere else for plastic surgery. People are shocked when they see how big I used to be. Stellar stellar work.
Can you please clarify which images are what? Before/after/simulation? It is not clear from the photos presented with your case.
It appears that you need some type of alar support (articulated alar rim grafts, specifically). As the nose heals, there is always some element of alar retraction/notching. In thicker skinned individuals, this is particularly important to support the very heavy soft tissue envelope because that will contract/change over time. I would also recommend an alar base reduction to narrow the ala (nostrils).
I would recommend a rhinoplasty. There is no better way to change the shape/structure/function of the nose.
I would ONLY recommend surgery; any non-surgical intervention would surely not address the issues correctly. I would recommend a revision rhinoplasty with rib graft. The "ball" that you see in the tip is likely a combination of the tip cartilages sutured together over or next to some type of graft (usually a septal extension graft). In your particular scenario, it looks like maybe a shield graft was placed over the tip as well. Over the years, the skin will continue to thin and contract around the cartilage frame work.
I would recommend facelift/necklift with osseous genioplasty (advancing the chin bone). Another option would be facelift/necklift with a chin implant.
