Breast implant downsize - Will I need a lift? What can I expect from scarring? (Photos)

Over 10 years ago I got breast implants. I went from a natural A cup to silicone implants that brought me to a C cup. I am 5 4", 112 lbs. I now feel they are too big for my frame and are causing me back problems. I want to get a revision and go down to a B cup. Will I need a lift as well and does that include skin removal and an extra scar?

Thank you for your question and photos. Based on these photos, you are correct, these implants are too large for your frame. They stretched your tissues below the inframammary folds (your scars moved up on the lower poles of the breasts as the implants kept going south), and they tended to lateralize into your armpits. Obviously, there are multiple approaches trying to improve your breast aesthetics and proportions. These issues need to be discussed during an in person consultation. My personal recommendation would be to remove the implants and allow your breast tissues to recover and rebound from being over stretched. In 4-6 months, you could do well by placing smaller implants without needing a lift, which does require additional scars besides the ones you have. See a board certified plastic surgeon experienced in all aspects of breast surgery for an in person consultation/evaluation. Good luck.

Based on your photos, downsizing your implants is certainly possible. Whether you need a breast lift depends on your skin elasticity, nipple position, and how much excess skin remains after implant removal. A modest reduction may not require a lift, but if there is noticeable skin laxity, combining a mastopexy with implant exchange can provide a more youthful shape. A lift does involve additional skin removal and permanent scars, although these typically fade with time. An in-person consultation will determine the best approach for your goals.
