I had a revision augmentation & lift (went from OTM-Subfacial) in February 2026 and when i lie flat on my back, my breast seem to be migrating more and more towards my armpits leaving my chest wall completely open. I also supposedly have galaflex mesh which was to help with support and position of the inplants. I also have rippling in the cleavage area which I did not have with UTM, but get that I’m thin. Had a phone follow up with the surgeon and he says everything looks great. Not feeling great, it looks terrible. Should I be going back to OTM or can the pocket be tightened to help with the migration to my armpits? Thanks

Thank you for your question. I have been in practice for over 30 years and revisionary breast surgery is a large portion of my practice.  I rarely place implants on top of the muscle.  The fascia of the pectoralis major muscle is thinner than tissue paper, therefore subfascial is the same as subglandular.  You virtually have no support.  Gala flex mesh or any other kind of mesh gives a very little support because the whole breast mound will move with the mesh included.  Your best option is to go under the muscle and revise the lift.  This will get rid of the medial wrinkling and also lateral displacement of your breast implants.  Best wishes to you.
