Does my nose bone need to be broken for straightening the nose, as my right nostril is 80% blocked. Do I need a septoplasty or a regular rhinoplasty? I am reluctant to having a procedure under general anesthesia, could I have it done under local? What exact procedure would I need, considering my nose looks different from each angle.

In order to fix your breathing you have to straighten your septum with a proper septoplasty and potential functional aesthetic rhinoplasty. General anesthesia is the best option for this but there are other options including IV sedation. It depends on your goals!

Answer: Septo rhinoplasty with osteotomies maybe necessary to achieve a straighter nasal appearance. Since the nose is part of the airway, we recommend general anesthetic for everybody. And since the inside of the nose is very sensitive, I do not believe that it is feasible to have septorhinoplasty performed with strictly local anesthetic. The purpose of osteotomies would be to achieve a narrower and straighter nasal appearance. The purpose of septoplasty may be to help you breathe better through the right side of your nose and to harvest grafting material that might be necessary to achieve a straighter nasal appearance. It will require straightening of the septum as well as straightening of the nasal bones, middle vault, and tip. I perform all rhinoplasty under general anesthesia. There are many rhinoplasty surgeons who perform rhinoplasty with heavy sedation. If your heart and lungs are in good shape you should not have any reservations about having a general anesthetic. Postoperatively you should not expect your nose to look exactly the same from each angle. But you should expect a lot of improvement in your breathing and nose appearance. You are a good candidate for septorhinoplasty.
