I underwent a functional and aesthetic septorhinoplasty 7 months ago. The surgery was open and involved straightening the septum and fixing the turbinates. I am a 41-year-old woman with thin skin. After the surgery, the area swelled a lot and even after all this time, the swelling has not gone down. In my case, my upper lip (philtrum area) was very long, and now with the rhinoplasty, it has lengthened even more because the tip of my nose has been rotated upwards; it is quite upturned.
Answer: Revision rhinoplasty Hello and thank you for your question. if you are bothered by the rotation of your tip, this could be derotated during a revision by releasing scar tissue and placing extended spreader grafts. I recommend that you seek consultation with a qualified board-certified rhinoplasty surgeon who can evaluate you in person. Best wishes and good luck. Richard G. Reish, M.D. Harvard-trained plastic surgeon

Answer: You appear to have a pleasing nasal appearance after septal rhinoplasty surgery. Based on your photos, you started with an obtuse nose lip angle. Your tip may have been rotated upward a little bit, however, it is not a significant change from preoperative. If this is something that is bothering you a lot, the only way to lower your nasal tip would be to utilize a septal extension graft and extended spreader grafts. Another option would be to consider a lip lift surgery. Certainly wait a full one year until you make a decision whether or not you wish to undergo further treatments. Thank you for your question. Sincerely, Dr Joseph

Answer: Rhinoplasty The photo gives limited information but your nose looks pretty good. Swelling can be improved with a steroid injection. You might want to ask your surgeon about this

Answer: Early result after rhinoplasty It is impossible to give you fair evaluation of your result without a series of photos. Slightly rotated tip will correct itself in time and swelling of the tip will subside. It is unlikely that external rhinoplasty had much influence on upper lip length and position? Waiting at least one year for result to mature would be my recommendation. Good luck.

Answer: What is your question? To make a quality assessment regarding the outcome of any plastic surgical procedure, we need to see quality before and after pictures. If you don’t have before, and after pictures, then ask your surgeon to forward the pictures they took. At seven months, you are no longer swollen, and your outcome is the result of the procedure. if you have concerns about the outcome, then talk to your surgeon. To get a proper second opinion, consultation schedule those in person consultations with other plastic surgeons in your community. For second opinion consultations come prepared bring with you a complete set of proper before and after pictures and a copy of your operative report. Those are all available from your current providers office if you request them. Best, Mats Hagstrom MD
