Question

4 months post neck lipo with a board-certified plastic surgeon, 42 years. Very unnatural contour, look like my neck was scooped out with a melon baller. Cavity & numb under the chin, but no blending, so jowls hang. I look way worse than before and am too sad to let anyone see me.

There is no doubt that you will benefit from neck revision. However, consideration should be given to lower face lift as well as tightening of the posterior and lateral neck. This can not be achieved through anterior neck approach. Second opinion consultation with a board certified plastic surgeon with known expertise in face/neck lift is recommended. Good luck.

Thank you for sharing your question and photographs.  Yes this is a cobra neck deformity and will require a full neck and likely facelift to improve upon the current appearance.  be sure to seek out a specialist in consultation as this correction will be difficult.

What you have is a cobra neck deformity and multiple issues in your neck anatomy which include over resection of the central fat pad, platysmal bands, loose skin, and probable hypertrophy of the submaxillary glands in addition to residual lateral fat in the neck.  A lower face and neck lift procedure is going to be required to accomplish tightening loose facial and neck skin, tighten loose facial neck muscles which includes a platysma plasty  to improve the  contour of the lower neck area

The main issue here is that with the excess skin, liposuction alone was not the appropriate first procedure. Your doctor should have prepared you for that, if they didn’t. The only way to correct this now is with a full necklift with incisions behind the ears. Best of luck.

Sorry for your problem and you would appear to benefit from a formal necklift. So go on some complimentary consultations with experts and compare recommendations after a full examination and discussion of desired results and look at photos of similar patients and become fully informed about the desired procedure. It might also help to get your preop photos and records from the previous procedure

This is not a skin or a jowl problem, a routine necklift will not help this.  What you are seeing is the digastric muscle and the submandibular gland hanging into the midline space from which presumably too much fat was removed.

Correction of this problem involves removing both gland and muscle; it is not a beginners operation.   There are at least 2 very competent plastic surgeons in SF who are worlds experts in this problem
