Dr who did the SmartLipo 8 mos ago recommended Tempsure Envi. I did Tempsure 30 days ago w/ no improvement. 2nd opinion Dr suggested Morpheus 8. I don't want to spend thousands more on minimally invasive treatments. I want this fixed! I realize the necklace lines are just who I am but I feel they are greatly emphasized by what is either remaining fat or skin sagging. I also have these "jowls" that make me look like I'm older/heavier/mad. I want a defined neckline. Would real surgery help me?
Answer: Necklift and chin implant Only a neck lift will solve your concerns. You need a more defined jawline, liposuction, and defined neck which only a necklift can do. However, your horizontal lines may not be helped much if that's your concern. Its best to have a personal consultation.

Answer: Lower face and neck lift with chin implant or fat transfer and necklace lines injection I believe you would be a great candidate for a lower face and neck lift, and your 40s is a wonderful age to consider getting one. Aging is a part of life and by addressing these concerns now you will be able to continue to age gracefully without having to go through an intense series of treatments later in life. While 40 may seem young, I have plenty of patients in that age range that come in to consult for a face lift because it allows for you to look refreshed without anyone having to notice an immensely drastic change in your appearance. A lower face and neck lift would address the sagging around your neck and jowls and allow for a youthful rejuvenated look without looking too “done.” I would also suggest to address your underdeveloped chin, which is drawing more attention to the sagging around your jowls and neck. While addressing your underdeveloped chin is not necessary, I believe creating more chin projection would really bring out the features around your jaw and neck and would create a well balanced look within your face. If the necklace lines are something that bother you, the appearance of them can also be softened either during surgery or later down the line with a treatment that I like to perform which involves injecting diluted hyaluronic acid filler directly into the necklace lines. The injections are very targeted which helps to fill the hollow spacing without creating any bulkiness to the area. Going forward, I would recommend you to complete multiple in office consultations with board certified plastic surgeons to discuss your ideal outcome. Answer: Improved Neck Contour Your photos shows a small (recessive) chin, which blends into your neck, with the neck angle blunted by fat and probably a low hyoid bone. The lower jowl bulges in your lower cheeks crossing the jawline camouflage a defined jawline. I think a lower cheek and necklift would allow surgical removal of fat in the neck and lower jowls, along with tightening of neck muscles. I would also recommend a chin implant to add better balance to the lower face. Possibly some microfat grafting to your horizontal lines. All of this would create a more defined jawline, a sharper neck angle, and a more pleasing, youthful look.
